A collection of 2,278 isolates belonging to 86 different fungal species was tested with micafungin and eight other drugs using the EUCAST procedures. Micafungin was active against species of Candida and Aspergillus (even azole-resistant species) as well as Penicillium spp., Scedosporium apiospermum, and Acremonium spp. It was inactive for species of Basidiomycota and Mucorales and for multiresistant species such as those of Fusarium.Micafungin is a new drug that belongs to the echinocandin class of antifungal agents. Its mechanism of action is by means of the inhibition of 1,3-␤-D-glucan synthesis in the fungal cell wall (10).Micafungin has been recently approved in Europe and the United States for the treatment of candidemia, acute disseminated candidiasis, Candida peritonitis and abscesses, esophageal candidiasis, and recently for the prophylaxis of Candida infections in patients undergoing hematopoietic stem cell transplantation.The in vitro activity of micafungin against most common species of Candida is well known (4, 11-13). However, information is limited for uncommon species of yeasts as well as for molds.The aim of this study is to analyze the in vitro activity of micafungin and eight other antifungal agents against a collection of clinical isolates of yeasts and molds from human beings using the methods approved by AFST-EUCAST.The strains were recovered from 115 Spanish hospitals through a period of 3 years, from 2005 to 2007. A total of 2,278 clinical isolates were included in the analysis. Isolates were identified by morphological and biochemical methods and sequencing of DNA targets if necessary. They belonged to 86 different species of common and emerging fungal pathogens. The isolates were obtained from blood (559; 24.5%), biopsies and other deep sites (217; 9.5%), respiratory tract specimens (751; 33%), skin samples (180; 7.9%), and other locations (707; 25.1%).The following drugs were used: amphotericin B (range, 16.0 to 0.03 g/ml; Sigma-Aldrich Quimica S.A., Madrid, Spain), flucytosine (64.0 to 0.12 g/ml; Sigma-Aldrich), fluconazole (64.0 to 0.12 g/ml; Pfizer S.A. Candiduria is a common infection of the urinary tract. Evaluation of the susceptibility to antifungal drugs could be used for treatment and control of infection.Background: Candiduria is one of the most common symptoms of urinary tract infections caused by several species of Candida spp.. Several antifungals are available to treat such candidal infections. During the last decades, resistance to antifungal especially to non-albicans species has increased. Objectives: The present study aimed to evaluate the susceptibility to antifungal drugs of Candida species isolated from candiduria in Ahvaz. Materials and Methods: Ninety three species of yeasts and yeast like organisms isolated from urine samples [Candida albicans (58), C. glabrata (25), C. tropicalis (4), C. krusei (1), unknown Candida species (4) and Geotrichum species (1)] were used for susceptibility tests. All species were re-identified based on standard mycological methods. Then a suspension of each isolate of overnight cultures was prepared in 1ml of sterile PBS and adjusted to 0.5 McFarland turbidity standards. In the present study several antifungal drugs (fluconazole, amphotericine B, ketoconazole, econazole, itraconazole) were used for susceptibility test using disk diffusion method. Results:In the present study all tested isolates were sensitive/dose dependent to amphotericine B and nystatin, whereas only one isolate of C. glabrata was resistant to both antifungals. Resistance against fluconazole (48.4%) and ketoconazole (26.9%) were observed among tested isolates. Resistance against fluconazole was detected among all tested organisms, 34.4% of C. albicans, and 7.5% of C. glabrata. On the other hand, all isolates were sensitive to econazole (93.5% sensitive, 6.5% dose dependent). Conclusions: It was concluded that Candida species isolated from candiduria in hospitalized patients had excellent in vitro sensitivity against econazole. Other suitable antifungal drugs were amphotericine B and nystatin, itraconazole. Whereas, resistance against ketoconazole (26.9%) and especially fluconazole (48.4%) was significant.

Children with urinary tract infection present with vague clinical features and candiduria in such patients signifies pyelonephritis/cystitis, disseminated candidiasis, contamination or colonisation. This study was aimed to determine the prevalence of candiduria in paediatric patients, their speciation, associated risk factors and antifungal susceptibility of these isolates which will help the clinicians in the better management of candiduria. In our study, the isolation rate for candiduria was 24.3%, more common in males (63%) compared to females (37%) and infants were most commonly involved. Colony count of ≥10 5 cfu/ml seen in 53% of the isolates and pus cells were seen in 84% cases. Among candiduria cases, 87% received broad spectrum antibiotics for more than 7 days, while 57% were catheterized, and 19% of the patients were on fluconazole therapy. In the present study, non-albicans candida species emerged as the predominant pathogen and was found in 57% cases while Candida albicans was found in 43% cases of candiduria. Antifungal susceptibility testing revealed that all the isolates were sensitive to voriconazole and ketoconazole while 95% of the isolates were sensitive to fluconazole and 92% were sensitive to itraconazole. Resistance to fluconazole was found in 5% of isolates while 8% isolates were resistant to itraconazole. Identification of candida species along with antifungal susceptibility is important in patients with UTI and will help the clinician in selecting the appropriate antifungal agent for better management of such cases.
