Survey of Current Practices among Members of CAG in the Follow-Up of Patients Diagnosed with Gastric Ulcer

Abstract: Physicians vary widely in their follow-up of benign gastric ulcer. Studies on the occurrence of gastric cancer in this setting are not unanimous in their conclusions. Subgroups of patients with NSAID exposure and successfully eradicated H pylori infection may have a lower risk of malignancy. Studies to confirm this are warranted, and modified guidelines may be appropriate.

“…Regardless of the guidelines, surveillance rates of Canadian and American endoscopists range from 16.0% to >60% (31,32). Although the annual age-standardized gastric cancer incidence rates per 100,000 are 65.9 (33) in Korean men vs. 7.8 (34) in nonHispanic Caucasian men, only one cancer was found among the endoscopically benign ulcers in the current study, which had atypia on the first biopsy.…”

“…14 Although no U.S. or Canadian gastroenterologic society has recommended routine surveillance for gastric ulcers, it is a common practice. 22 A recent analysis of the Clinical Outcomes Research Initiative database found that approximately 25% of patients diagnosed with gastric ulceration undergo repeat upper endoscopy within 3 months despite the fact that multiple studies have found limited yield in identifying malignancy with surveillance endoscopy. 23 The decision to perform surveillance endoscopy in patients with a gastric ulcer should be individualized.…”
