We report the third presentation of an intermixed arteriovenous malformation and hemangioblastoma. The rare occurrence of the diagnostic histologic features of both a neoplasm and vascular malformation in a single lesion is more common in gliomas, as angioglioma, and is termed an 'intermixed’ lesion. We review the literature concerning the developmental biology of each lesion, and potential interplay in the formation of an intermixed vascular neoplasm and vascular malformation. The roles of cellular origin, genetic susceptibility, favourable microenvironment, altered local gene expression and key regulatory pathways are reviewed. Our review supports angiography and genetic profiling in intermixed lesions to inform management strategies. Consideration should be given to multimodality therapeutic interventions as required, including microsurgical resection, stereotactic radiosurgery and further research to exploit emerging molecular targets.

Craniopharyngiomas (CPs) are rare benign epithelial tumours. Brain arteriovenous malformations (AVMs) are also rare lesions occurring in young adults. The appearance of both these lesions in the same patient is rare. A 42-year-old patient presented with headaches for 3 months and a progressive decrease in his visual acuity. Bitemporal hemianopsia was detected in the visual field. Magnetic resonance imaging (MRI) revealed a tumour with cystic and solid components located in the suprasellar region and AVM in the right temporal lobe. AVM (Martin–Spetzler grade III) was visualised using digital subtract angiography (DSA), which was fed from the right middle cerebral artery and drained through the sigmoid sinus via the inferior petrosal sinus. The patient was operated with enlarged right frontotemporal craniotomy. AVM nidus was totally removed at the first operation. Embolisation was not preferred before the AVM surgery. After 3 days, sylvian dissection was performed using the same craniotomy. The tumour was completely removed via the carotid cistern by making sharp dissection from the infundibulum. Post-operatively, the patient showed normal neurological examination and significant improvement in his visual field examination. There was no residual/recurrent tumour or AVM on contrast-enhanced MRI and DSA at post-operative 6 months. Histopathological examination revealed AVM in the first operative material and papillary-type CP in the second. The coexistence of these two rare pathologies has previously been reported in only one patient. This is the first case of surgical resection of CP and AVM using the same craniotomy.

A 25-year-old man admitted to our department with chief complaints of headache and giddiness for last 2 weeks. Two weeks back the patient had history of headache of sudden onset, severe in intensity with sudden loss of vision. He regained his vision without any deficit within minutes. There was no history of unconsciousness, seizures or focal deficit, and persistent diminution of vision. On examination the patient had visual acuity 6/6 in the both eyes with no field defects. On fundus examination no abnormality was detected. Hormone analysis showed level of prolactin greater than 200 ng/mL.Contrast-enhanced computed tomography (CECT) of the brain (►Figs. 1 and 2) on admission showed welldefined, contrast-enhancing, hyperdense lesion of size 24 Â 20 Â 17 mm. Lesion seen in parafalcine region of the right parietal lobe associated with prominent perilesional vessels appeared draining in superior saggital sinus suggestive of arteriovenous malformation (AVM). There is evidence of widening of sella with presence of soft tissue density lesion of size 20 Â 17 Â 20 mm pituitary adenoma.Magnetic resonance imaging (MRI) of the brain and sella showed a mass lesion in sella showing presence of solid and cystic component. The cystic component appeared hyperintense on T2 whereas solid component appeared isointense to gray matter on T1 and T2W images. Hemorrhage was noted within lesion showing fluid level and hypointense on T2W images. Mass was seen extending into the sphenoid sinus. A small AVM was noted along medial aspect of right parieto-occipital lobe showing presence of multiple vascular channels and early draining vein into the superior saggital sinus and into deep venous sinuses. There was associated mild perilesional gliosis. It showed heterogenous enhancement on postcontrast enhancement.Cerebral CT angiography showed AVM with nidus measuring 25 Â 22 mm in parafalcine region of the right high parietal lobe with the caliber of draining vein measuring 4 mm draining into superior saggital sinus.
