Intermixed arteriovenous malformation and hemangioblastoma: case report and literature review

Abstract: We report the third presentation of an intermixed arteriovenous malformation and hemangioblastoma. The rare occurrence of the diagnostic histologic features of both a neoplasm and vascular malformation in a single lesion is more common in gliomas, as angioglioma, and is termed an 'intermixed’ lesion. We review the literature concerning the developmental biology of each lesion, and potential interplay in the formation of an intermixed vascular neoplasm and vascular malformation. The roles of cellular origin, ge… Show more

“…The vessels that make up this malformation are dilated and dysplastic. They are jointly referred to as the nidus, which is connected with feeding arteries and drainage veins without the presence of brain parenchyma cells interspersed between the nidus vessels [ 5 , 42 , 43 , 44 ]. These connections allow AVMs to exert circulatory arrest on the surrounding brain structures [ 45 ].…”

“…An AVM nidus bears a resemblance to an HBM nodule due to dense collection of vessels [ 14 ]. Furthermore, HBMs histologically comprise numerous capillary channels, but an AVM nidus is made up of dysplastic arteries and veins, with the characteristic absence of an intervening capillary bed [ 15 ]. Because HBMs are associated with a coexisting feeding artery aneurysm, when hemorrhage is induced by HBMs, a cerebral aneurysm should also be considered [ 16 ].…”

“…Although a rich vascular supply, mural nodule and cyst formation are frequently seen in the tumor of hemangioblastoma, other vascular anomalies near or adjacent to the tumor are not frequent. Several reports have been published as case studies in the literature [ 1 - 4 , 8 - 10 ] (Table 1 ).…”

“…Treatment with radiosurgery was often employed with a favorable tumor control for residual tumors. A few cases have been reported who developed hemangioblastoma in association with vascular anomalies [ 1 - 7 ]. We have encountered a case of hemangioblastoma associated with an arteriovenous malformation (AVM) near the tumor, though they were located separately below and above the tentorium.…”
