Surgical site infections in HIV-infected patients: Results from an Italian prospective multicenter observational study

Abstract: Although 92.5% of our HIV-infected patients presented a NNIS score < or = 1, the SSI rate was twofold higher than that reported in Italian and European studies for the general population, with more severe clinical presentations. This is the first report of an association between HCV-HIV co-infection and SSI occurrence. Additionally, the viro-immunological status of our patients was not related to SSI occurrence, which suggests the need for further research for other potential risk factors that may be implicate… Show more

“…12 Diabetes mellitus, a predominant co-morbid condition was observed in 11.5% of SSI patients as compared to an SSI rate of 8.8% in non-diabetic patients which is statistically insignificant. This result is not consistent with literature quoting between 20 to 33% SSI rate in diabetic patients.…”

“…These results are consistent with literature except for one study by Lilani SP et al where pseudomonas was found to be the commonest organism, Staphylococcus aureus being the commonest commensal and organism responsible for SSI logically conveys the consistency with literature. 4,9,12 The main limitation of the present study is that it is just a longitudinal study with within group comparative analysis. No formal sample size calculation was carried out due to multiplicity of the factors under study.…”

“…Poor control of glucose during surgery and in the perioperative period increases the risk of infection and worsens outcome from sepsis. Hypertension, preexisting or pregnancy induced, HIV, and other comorbid states have been associated with SSI in several studies [13,21,34,35]. They are associated with low vitality and thus predispose to infection [21].…”

“…Surgical site infection is the third most frequently reported nosocomial infection and one of the main factors causing patient mortality after surgery (15). The mortality rate of patients with sepsis is 30% to 40% and up to 50% for those with severe sepsis or septic shock (1,16). In an attempt to reduce the postoperative complications in HIV-patients, a recent study reported specific guidelines for treatment (17 (Tables 5, 6) since 6 of the 10 patients (60%) ( Table 4) with complication had more than five points to the charlson score, this wasn't an absolute relationship since 3 (3/25, 12%) patients with charlson score 6, 9, and 7 respectively had no complication and Clavien 0, and also 2 patients with Charlson score 2 and 3 that had a complication with Clavien 2.…”

“…From the first time that human immunodeficiency virus (HIV) was discovered, till today both the quality of life and survival expectancy of HIV-infected patients have markedly improved (1). As the life expectancy of these patients increases due to the use of highly active antiretroviral therapy (HAART) also increases the number of HIV-positive patient to be subjected to an operation (2).…”
