Surgical removal of subfoveal choroidal neovascularisation in highly myopic patients

Abstract: Conclusions-The treatment of subfoveal CNV in highly myopic patients by surgical removal by vitrectomy does not achieve any significant improvement of the BCVA. (Br J Ophthalmol 2001;85:1041-1043 High myopia (more than −6 dioptres and/or longer than 26 mm of axial length), is the seventh most common cause of legal blindness in adults in the USA.

“…Several other treatments have been assessed, including thermal laser photocoagulation [6], macular translocation [7,8], surgical removal of CNV [9], radiotherapy [10], indocyanine green mediated photothrombosis [11,12], and combined PDT and intravitreal triamcinolone injection [13,14]. However, the beneficial effects of these treatments are questionable, primarily because of the risk of severe complications or poor long-term results.…”

“…Controversial results were reported in submacular surgery for myopic CNV [132,133,134] ranging from no change [134] to an improvement of 2 lines in 45% of the eyes [132,135]. Two techniques of macular translocation – limited macular translocation and macular translocation with 360-degree (MT360) retinotomy – have been performed in the last decade also with controversial results [136,137,138,139].…”

“…These include thermal laser photocoagulation, 14 photodynamic therapy (PDT), 15 macular translocation, 16 and surgical removal of CNV tissue. 17 The frequency of CNV complications undermines these treatments and generally bears poor long-term results.…”
