Surgical Management of Patellofemoral Instability in the Skeletally Immature Patient

Abstract: Compared with skeletally mature patients, skeletally immature patients are at a higher risk of acute traumatic patellar dislocation. Surgical treatment is the standard of care for patients with recurrent instability and requires important and technically challenging physeal considerations. Physeal-sparing medial patellofemoral ligament reconstruction is the treatment of choice for these patients, replacing older nonanatomic extensor mechanism realignment techniques. Implant-mediated guided growth is an importa… Show more

“…Recognition and treatment of PFI have undergone considerable evolution in recent years, with more than 100 different surgical techniques reported in the literature [ 72 , 73 , 74 , 75 , 76 ]. In patients with a first-time dislocation without significant risk factors for recurrence and no osteochondral fracture, non-operative management is recommended [ 9 ].…”

“…65 Overall, operative intervention has been shown to result in adequate patient outcomes, satisfaction, and postoperative stability. 16,18,58,63,68,88 Improved knowledge about pathologies involving the patellofemoral joint has been a major driving force for tailored management. 90 With the increase in treatment approaches and research on patellar instability, it is imperative to analyze the types and quality of these investigations to determine the driving factors for current and future clinical practice.…”

“…26 Repair should only be considered for patients with an initial patellar dislocation who are undergoing a concomitant procedure (eg, osteochondral repair) because MPFL repair has been shown to result in just as many recurrent dislocations as nonoperative management and more recurrent dislocations than MPFL reconstruction. 31,35 MPFL injuries can occur at the femoral insertion, midsubstance, patellar attachment, or more than 1 of these sites. 11,15 MPFL injuries at the femoral attachment tend to be more common in adults, while MPFL injuries at the patellar attachment tend to be more common in children and adolescents.…”
