Zusammenfassung. Im Beobachtungszeitraum von 2 Jahren (im Mittel 15 Monate) wurde an 120 Augen von 98 Patienten wegen eines therapieresistenten Glaukoms mit dem Diodenlaser eine transsklerale Zyklophotokoagulation (TSCPC) durchgeführt (G-Sonde, Iris Medical Oculight SLx).Bei 76,5% der Patienten war der Augendruck bereits nach einer Operation postoperativ definitionsgemäß reguliert, 13 Patienten benötigen einen Zweiteingriff, bei 4 Patienten war eine dritte Behandlung erforderlich.Folgende postoperative Komplikationen wurden beobachtet: iritische Reizzustände bis zu Fibrinreaktionen, Vor -d e r k a m m e r b l u t u n g e n und Aderhautabhebungen; sie waren konservativ zu beherrschen. Ein zystoides Makulaödem führte bei 2 Patienten zu einer Verschlechterung des Sehvermögens von mehr als 2 Zeilen.Eine Phthisis bulbi war bei 2 Augen zu beobachten. Neben der recht hohen primären Erfolgsrate und der -im Vergleich zu anderen Methoden -geringeren Komplikationsrate spricht auch die Möglichkeit mehrfacher Wiederholungen für dieses zyklodestruktive Verfahren.Schlüsselwörter: Zyklophotokoagulation, transskleral, Diodenlaser, therapieresistentes Glaukom, cystoides Makulaödem.Transscleral cyclophotocoagulation with a contact diode-laser for the treatment of refractory glaucoma Summary. Over a period of 2 years (mean follow-up time of 15 months) 120 eyes (98 patients) with medically untreatable glaucoma of different ethiology were treated with transscleral contact diode laser cyclophotocoagulation (TSCPC) using the Oculight SLx diode laser system with the G-probe delivery.According to our definition in 76,5% of the eyes the intraocular pressure (IOP) was adequately controlled over the period of observation. 13 patients were in need of a second operation, in 4 patients a third procedure was performed.Postoperative findings included anterior segment inflammatory signs ranging from a mild flare in most cases to a severe iritis with fibrin exsudate in a few, hyphema and chorioidal detachment. Most cases responded well to medical treatment. Rare complications are also described: cystoid macular edema was observed in three patients with significant decrease of visual acuity of more than 2 lines in two of them. A phthisis bulbi occurred twice over the follow-up period.When compared to other cyclodestructive procedures TSCPC offers a high rate of primary success with relatively few severe complications in otherwise untreatable cases of glaucoma. Surgery time is short and the procedure can easily be repeated if neccessary.
