Surgical Correction of Abdominal Testes after Fowler-Stephens Using the Neodymium: YAG Laser for Preliminary Vessel Dissection

Abstract: Two boys with high undescended testes were submitted to laparoscopic laser transsection of the internal spermatic vessels. Subsequently strong collaterals developed. Definitive scrotal transfer was performed six weeks later through an inguinal incision. Two and a half years later, the testes had recovered very well. Their volume had increased markedly, and further development was normal.

“…Many procedures can be used (6,7,10,22). Among them, laparoscopy is very useful for performing a two-step Fowler-Stephens procedure (1,3,18), In this series, twenty-three testes underwent a two-step orchiopexy with laparoscopic ligation of the spermatic vessels. Fifteen children were controlled six months or more after the second step, Fourteen testes were in the scrotum and had a normal consistency and size (93 %) while one was atrophic, Lastly, one hundred and ten cases (forty-seven per cent of this group) could benefit from laparoscopy.…”

“…3, 4), including a bilateral exploration (the incision is easily extended into a Pfannenstiel). In our opinion, the only real advantage of laparoscopy is that it enables the surgeon to perform a two-stage Fowler-Stephens operation with laparoscopic clip-ligation of the spermatic vessels as first stage [26][27][28], In this event, the testicle can be brought down to the scro- …”

“…EGT ise inmemiş testisler içerisinde %12-24 oranında rapor edilmiştir (4,5) . EGT'te ilk defa yapılan laparoskopi deneyiminden sonra laparoskopi giderek daha çok ön plana çıkmıştır (6)(7)(8) . Radyolojik görüntüleme ve ultrasonografi EGT'lerde kullanılabilir, ancak oldukça sınırlı sayıda hastada yarar sağlayabilmektedir (9) .…”
