Surgical ciliated cyst of the mandible after orthognathic surgery: a case report with review of the literature

Abstract: Background Surgical ciliated cysts, also known as postoperative maxillary cysts or implantation cysts, occur mainly in the posterior maxilla after radical maxillary sinus surgery; they rarely develop in the mandible. They are thought to occur when the sinonasal epithelium is infiltrated by a surgical instrument during surgery or as a result of transplantation of bone or cartilage with respiratory epithelium attached. Case presentation We report a c… Show more

“…Over time, these trapped cells can undergo cystic degeneration and, subsequently, expansion caused by the osmotic difference with locoregional tissue (11). In the mandible, respiratory epithelium transplantation has been associated with the use of nasal osteocartilage graft and maxillary bone graft used to stabilize genioplasty (12,13). These cysts could also arise from accidental transport of the sinus mucosa on the saw blade during bimaxillary surgeries (14).…”

“…Only 12 such clinical cases have been reported. 9 Treatment of a surgical ciliated cyst involves simple enucleation, as with POMC, and shows a low recurrence rate. Since all related surgeries were denied according to the patient, the causes of the lesions in the mandible with subsequent recurrence remain unclear.…”

“…The surgical ciliated cyst, which is pathologically very similar to POMC, is very rare and typically occurs in areas of the maxilla that have experienced previous surgical intervention (orthognathic surgery, rhinoplasty, or plastic surgery). Only 12 such clinical cases have been reported 9. Treatment of a surgical ciliated cyst involves simple enucleation, as with POMC, and shows a low recurrence rate.…”

“…Ciliated cyst development in the mandible is exceptionally rare, with fewer than 20 cases reported to date [27]. Simultaneous surgical procedures involving both the maxilla and mandible, utilizing the same instruments, have been implicated in the mandibular PSCC development [28].…”
