Surgery and adjuvant radiotherapy vs concurrent chemoradiotherapy in stage III/IV nonmetastatic squamous cell head and neck cancer: a randomised comparison

Abstract: We compared concurrent combination chemotherapy and radiotherapy with surgery and adjuvant radiotherapy in patients with stage III/IV nonmetastatic squamous cell head and neck cancer. Patients with non-nasopharyngeal and nonsalivary resectable squamous cell head and neck cancer were randomised to receive either surgery followed by adjuvant radiotherapy (60 Gy over 30 fractions) or concurrent combination chemotherapy and radiotherapy (66 Gy in 33 fractions). Combination chemotherapy comprised two cycles of i.v.… Show more

“…Improving LRC, overall survival, and patient quality of life have always been the goals of HNC treatment (46)(47)(48)(49)(50)(51)(52)(53)(54). However, some patients still die of their disease, particularly those with larger tumors.…”

“…4 However, there appears to be no survival advantage when compared with other forms of therapy. [4][5][6][7] Previous studies have reported complication rates associated with surgical salvage following attempted organ-preserving treatment for HNSCC at 39 percent to 61 percent. [8][9][10] When primary organ preservation therapy with concomitant chemoradiation is utilized, the role of surgery is not standardized.…”

“…We read the recently published article by Soo et al (2005) with great interest. They compared adjuvant radiotherapy following surgery with combined radiochemotherapy for locally advanced head and neck cancer.…”

“…They selected patients with resectable head and neck cancer for their trial, but it is highly probable that such patients prefer surgery. Therefore, most studies concerning radiochemotherapy of head and neck cancer deal with unresectable tumours (Soo et al, 2005). The authors thus owe their readers an explanation as to which criteria they applied in distinguishing resectable from unresectable tumours.…”
