Background: Adipokines are demonstrated to be associated with multiple cutaneous diseases. Leptin is mainly produced by the adipocytes that stem from the obese gene. In addition, it was reported that, secretion of leptin is a response to increased lipid uptake, thus, it might be regarded as a link between improper diet and the development of inflammatory acne. Objective: The aim of the current work was to estimate serum leptin and adiponectin in both obese and non-obese patients with acne vulgaris and to evaluate adiponectin/leptin ratio (A/L) rates as a biomarker of insulin resistance and hence their role in pathogenesis of acne vulgaris in correlation with body weight and disease severity. Patients and methods: This prospective case-controlled study included a total of 60 patients with acne vulgaris, attending at the Dermatology, Andrology & STD Outpatient Clinic, Mansoura University Hospitals. Forty healthy subjects matched with the patients in age, sex were included. This study was conducted between April 2019 to January 2020. Results: Cases with acne vulgaris demonstrated significant increase in serum leptin level as well as significant decrease in serum adiponectin level compared to controls. No significant correlation was reported between both serum leptin and adiponectin levels and disease severity. Leptin could be used as reliable predictor in terms of the differentiation between cases of acne vulgaris and controls with high sensitivity, specificity and accuracy. Adiponectin could be used as reliable predictor in terms of the differentiation between cases of acne vulgaris and controls with high sensitivity, specificity and accuracy. Conclusion: Acne vulgaris was associated with significant elevation in leptin level, significant reduction in adiponectin level and significant decrease in A/L ratio. Thus, leptin, adiponectin and insulin resistance may be pathogenic cofactors contributing to the development of the disease and could be used as reliable predictors for development of acne vulgaris but not for severity of disease.

Background and Aim: Recent data draw attention to the effect of body composition, insulin resistance and adipocytokines to acne vulgaris (AV) development. The aim of this study to assess the association of AV with insulin resistance and adipocytokine levels and to evaluate the effect of isotretinoin on insulin resistance and adipocytokine levels. Material and Methods: In 29 AV patients and 29 healthy volunteers, body mass index (BMI) and body fat mass (BFM), lipid, adiponectin, leptin, resistin, retinol binding protein-4 (RBP4) and insulin levels were measured and insulin resistance was assessed by HOMA-IR index in serum samples taken twice from patients before and after isotretinoin treatment. Results: In AV patients, pretreatment HOMA-IR and adipocytokine level was not found to correlate with disease severity. With five months of isotretinoin treatment, higher HOMA-IR values were found (p=0.028). Isotretinoin therapy maintained lower mean resistin levels (p = 0.016), higher mean RBP4 levels (p = 0.040), but not affected the mean adiponectin and leptin levels (p = 0.113, p = 0.125, respectively). Conclusions: All data suggests that five months of isotretinoin therapy in AV patients causes insulin resistance and the increase in insulin resistance is not dependent on age, BMI, BFM and lipid levels of these patients. Adipokines are the primary mediators of adipose tissue-induced and regulated systemic inflammatory diseases; however, recent findings revealed that serum levels of various adipokines correlate also with the onset and the severity of dermatological diseases. Importantly, further data confirmed that the skin serves not only as a target for adipokine signaling, but may serve as a source too. In this review, we aim to provide a complex overview on how adipokines may integrate into the (patho) physiological conditions of the skin by introducing the cell types, such as keratinocytes, fibroblasts, and sebocytes, which are known to produce adipokines as well as the signals that target them. Moreover, we discuss data from in vivo and in vitro murine and human studies as well as genetic data on how adipokines may contribute to various aspects of the homeostasis of the skin, e.g., melanogenesis, hair growth, or wound healing, just as to the pathogenesis of dermatological diseases such as psoriasis, atopic dermatitis, acne, rosacea, and melanoma.
