Objective This ex vivo study aimed to evaluate the strengthening effect of different ferrule and reattachment designs with fiber and adhesive materials on vertically fractured teeth. Methods Ninety extracted single-root premolars were instrumented and divided into nine groups (two control groups and seven experimental groups; n = 10). The negative control (NC) group comprised of intact teeth, while the positive control (PC) group comprised of root canal-treated teeth. The roots of the teeth in the experimental groups were vertically fractured into two equal fragments. The fragments were reattached with one of the followings: 4-methacryloxyethyl trimellitate anhydride/methacrylate-tri-n-butyl borane (4-META/MMA-TBB) resin, 4-META/MMA-TBB + quartz fiber post, 4-META/MMA-TBB + glass fiber bundles, 4-META/MMA-TBB + quartz fiber post + 1 mm ferrule, 4-META/MMA-TBB + glass fiber bundles + 1 mm ferrule, 4-META/MMA-TBB + quartz fiber post + 2 mm ferrule, and 4-META/MMA-TBB + glass fiber bundles + 2 mm ferrule. The core build-ups were made with composite resin. The specimens were subjected to compressive loading until failure occurred. Mean load necessary to fracture each sample and the fracture types of these samples were recorded. Results The highest mean fracture load was recorded in the NC group (1,036.7 N), which was not significantly higher than the PC group (989.66 N) (p > 0.05). The roots reattached with quartz fiber post demonstrated significantly less fracture strength (871.9 N) as compared to the other test and control groups (p < 0.05). There was no significant difference between the PC group and reattached fragments with different ferrule designs in terms of fracture resistance (p > 0.05). Conclusions The customized fiber bundles may be more suitable for reattachment of vertically fractured teeth than the rigid fiber posts. For reattachment procedures, the ferrule design may be preferred to increase the fracture strength of vertically fractured teeth.

Surgical extrusion is defined as the procedure in which the remaining tooth structure is repositioned at a more coronal/supragingival position in the same socket in which the tooth was located originally. Intentional replantation is defined as the deliberate extraction of a tooth and after evaluation of root surfaces, endodontic manipulation and repair, placement of the tooth back into its original position. Tooth autotransplantation is defined as the transplantation of an unerupted or erupted tooth in the same individual, from one site to another extraction site or a new surgically prepared socket. The advent of titanium implant rehabilitation has reduced the use of these treatments in day‐by‐day clinical practice; however, the re‐emerging trend to conserve and preserve natural sound tissues has led to a rediscovery of these treatments. All three distinct surgical methods are closely related, as they act to treat teeth that cannot be predictably treated using other more conventional procedures in endodontics, periodontics and restorative dentistry. Furthermore, these procedures share the same treatment approach and include the atraumatic extraction of a tooth, visual inspection of the tooth/root and its subsequent replantation. The clinical procedures for surgical extrusion, intentional replantation and tooth autotransplantation treatment have undergone several changes in recent years, and currently, there are no clear clinical treatment protocols/guidelines available. The clinician should be aware of the outcome of these treatments. Hence, the aim of this narrative review is to provide the background, clinical procedures and outcomes of surgical extrusion, intentional replantation and tooth autotransplantation.

The purpose of this case report was to describe the procedures and prognosis of intracanal adhesive treatment and intentional replantation for maxillary molar with crown/root fracture. The case of a vital tooth with buccal and lingual fragments caused by crown/root fracture was presented. The involved tooth was extracted and stored in balanced salt solution. Following confirmation of the repositioning of two fragments, the fractured surfaces of fragments were cut with a diamond bar until the cementum side remained. Finally, a large cavity was prepared in pulp chamber and the root canals like shell form. For the purpose of reinforcement after adhesion, a U-shaped orthodontic wire was bended which straddles the palatal canal and buccal canal. After drying the cavity wall, the dentin surface was treated with dentin conditioner, and the dual curing composite resin and the wire were filled in the cavity. After apicoectomy and retro filling, the tooth was replanted to original tooth socket. The prognosis was preferable at 1 month later and the tooth was used for an abutment of fixed metal bridge. The tooth still works in the mouth at the 3-year follow-up. Our results showed that intentional replantation after internal adhesive treatment with dual-cure resin and a metal wire is an effective and practical treatment for crown/root fracture of the maxillary molar.
