Successful Pregnancy Outcome in Women with Recurrent IVF Failure and Anti-hCG Autoimmunity: A Report of Three Cases

Abstract: We report three cases of effective management of infertility in women with a history of repeated unsuccessful IVF attempts, who have developed antibodies to hCG. A novel approach to conservative treatment of immunologic reproductive failure, suggested for selected patients, included membrane plasmapheresis, combined prednisolone, and intravenous immunoglobulin therapy. No adverse side effects were observed; all cases resulted in pregnancy and subsequent life births. In order to be given an adequate efficient t… Show more

“…Among all autoimmune diseases associated with POF, thyroid disorders are the most common [14]. It has been proposed that reduced fertility might be associated with the dysregulation of immune system reactions resulting in enhancement of autoantibody production [15]. This autoimmunity increases miscarriage risks, reduces female fecundity and infertility treatment success.…”

“…Cases of anti-hCG autoimmunity have been described, where patients have a history of repeated attempts to perform unsuccessful IVF treatments (144). The beta subunit of hCG is not immunogenic in women (145).…”

“…In this context, a structural alteration, due to a mutation in one of the beta-hCG genes and/or a functional abnormality of the immune system, could be implicated. In one case of infertility described with hCG immunization, pregnancy was achieved as a result of plasmapheresis treatment to detoxify the patient's body (144). Immunotherapy could be beneficial for patients who suffered from repeated implantation failures (RIFs) previously.…”

“…Four of them showed antibodies, but the neutralizing effect of hCG could be counter passed by increasing the doses of hCG used (75). The same kind of antibodies were described in women (77, 78). Immune response against hCG impairs fertility of women and induces pregnancy loss within the first trimester of pregnancy.…”

“…To thwart this negative effect, Muller and collaborators described a treatment that was successful in three women positive for hCG antibodies. This treatment combined membrane plasmapheresis, prednisolone, and intravenous immunoglobulin therapy (78).…”
