Successful management of heterotopic pregnancy after fetal reduction using potassium chloride and methotrexate

Abstract: Heterotopic pregnancy, the presence of two gestational sacs simultaneously, is a rare event but with the advent of Assisted Reproductive Technology, it is now an increasingly common complication. The reported incidence of a heterotopic pregnancy in a spontaneous cycle is quoted as 1 in 30,000. We report the case of a 38-year-old primigravida who was referred to our center at 11 + 2 weeks gestation with a diagnosis of heterotopic pregnancy for further management. A non-surgical intervention comprising of transv… Show more

“…Medical treatment would provide an effective choice for these patients [ 19 ]. It consisted of suction aspiration or a combination with injections of MTX, KCL in the gestational sac or fetal heart [ 20 ]. The use of MTX was contraindicated previously because of its potential embryotoxicity on the IUP but recent studies have proved its safety and effectiveness with local injection of low-dose [ 21 ].…”

“… 22 But evidence of MTX-related teratogenicity has already been observed in surviving intrauterine fetus after failed medical abortion or other treatment. 23 , 24 Though there are researches showed good therapeutic effect and no negative pregnancy outcomes with medical treatment of MTX, 7 , 16 , 25 , 26 ; we hold the attitude that the use of MTX, no matter systematically or locally, should be avoided in the treatment of HP.…”

“…The use of surgery via laparoscopy [ 5 7 ] and laparotomy [ 4 , 5 , 8 ] has been described as above. Administration of methotrexate and/or intracardiac potassium chloride has also been described in the past for treatment of abdominal pregnancies [ 6 , 10 13 ]. Not all of the prior reported cases of pregnancies in the uterosacral ligament trended beta-hCG levels [ 6 ].…”
