Successful doxycycline treatment of lymphatic fistulas: report of five cases and review of the literature

Abstract: Doxycycline can be used for treatment of persistent postoperative lymphatic fistulas in different locations. In further studies, this therapeutic concept should be evaluated with larger numbers of patients with regard to the ideal point of time, frequency, and mode of application.

“…They include drainage of the operative wound [6], external compression dressing [9], diathermy performed during surgery, ultrasound, argon, laser, harmonic scalpel [10], endoscopic procedures [11, 12], various techniques of axillary space closure [3, 13], fibrin glue [1420], sclerotherapy using tetracyclines [21] and tranexamic acid application [7, 9]. However, no standard resulting in effective diminution of SF incidence following axillary dissection has yet been established.…”

“…Topical PA‐MSHA injection therapy in our study was successful to cease the leakage of chylous fluid, and completely cured the chyle fistula. Similar to other sclerosing agents such as tetracycline, doxycycline, and OK‐432 (sapylin), the mechanism of PA‐MSHA to treat chyle fistula is thought that it initiates a strong local inflammatory response, which results in fibrotic adhesion between tissue surfaces and spontaneous fistula closure. Chyle drainage in all patients showed a marked decrease within 24 to 48 hours after the use of PA‐MSHA.…”

“…Cnotliwy et al [13] reported a complete fistula closure following administration of doxycyclin in 11 patients (median 3 days). Hackert et al [14] confirmed the successful use of doxycyclin in 4 out of 5 patients with fistulas at various locations. In dehiscent wounds complicated by persisting lymphocutaneous fistulas, Greer et al [15] reported the successful use of vacuum-assisted wound closure (V.A.C) which is currently widely used in surgical and, in particular, chronic wounds.…”
