Subungual pleomorphic fibroma

Abstract: Pleomorphic fibroma of the subungual region is an unusual cutaneous tumor with histologic features that may cause confusion with true sarcomas. This is only the second case reported of a subungual pleomorphic fibroma. Pleomorphic fibroma should be considered in the differential diagnosis of pleomorphic subungual tumors.

“…Two cases included in this series, although having the characteristic feature of OM, had, in our opinion, degenerative changes similar to those seen in cutaneous tumors with ancient changes, such as ancient schwannoma or pleomorphic sclerotic fibroma. 17 The patient was advised to follow-up carefully. Two years later, there was no sign of recurrence or spreading of the tumor.…”

“…In addition to these mononuclear cells, the atypical nuclear features can also be observed in multinucleated giant cells. [13] Degeneration, ischemia, or the paracrine influence of mast cells may create the cytological atypia in PF. [145] Two variants have been described: Myxoid and sclerotic.…”

“…Pathology revealed a circumscribed, hypocellular tumor in the papillary and reticular dermis with scattered, hyperchromatic, spindled, pleomorphic, multinucleated giant cells within haphazard collagen bundles and a mild lymphohistiocytic perivascular infiltrate. Tumor cells stained positive for vimentin and negative for smooth muscle actin, cytokeratin, factor XIIIa, and CD34 32. …”
