Subthreshold yellow laser for fovea-involving diabetic macular edema in a series of patients with good vision: effectiveness and safety of a fovea-sparing technique

Abstract: Purpose: Patients with center-involved diabetic macular edema (CI-DME) with good visual acuity (VA) represent a controversial clinical scenario in which a subthreshold laser might be a reasonable approach. We report a case series of patients with CI-DME with VA better than 20/32 who were treated with a subthreshold 577 nm (yellow) laser. Methods: The area of retinal thickening on OCT was treated with confluent laser spots at individually titrated power. The fovea was spared from treatment. Effectiveness and sa… Show more

“…Overall, a worsening of both BCVA and CMT was not observed in any of the cohorts evaluated [ 29 42 ]. In particular, a significant CMT decrease and BCVA improvement in two thirds were demonstrated [ 29 37 ] and in half of the studies [ 29 , 30 , 33 , 36 , 38 , 40 , 41 ], respectively, whereas a stabilization of these features was described in the remaining cases throughout the entire follow-up [ 31 , 32 , 34 , 35 , 37 42 ].…”

“…Besides the widely used OCT, also other image modalities may be helpful and contribute additional biomarkers. The quantification of short wavelength confocal 450 nm fundus autofluorescence, for example, may be considered as an imaging biomarker of retinal inflammation in DM, but this founding request further investigation of different fluorophores and their topographical distribution in the diabetic retina [ 170 , 171 , 172 , 173 , 174 , 175 , 176 ].…”

“…Предварительно для оценки зон отека и микроаневризм использовали ОКТ-А сканограмму, выполненную в режиме En Face по протоколу HD Angio Retina 6×6 мм на уровне ГСК, наиболее информативно отражающую данные изменения. Критерием включения был ОКТ-подтвержденный ДМО с интраретинальными кистами [17].…”
