Guest host complexes of β and γ cyclodextrins (CDs) with two spin labeled indole de rivatives having the same molecular weights but different structures were studied by EPR spectroscopy in aqueous solutions and semiempirical quantum chemical calculations of these systems were carried out. In the presence of CD the polarity of the NO group environment decreases and the rotational correlation time (τ) of guest molecules increases. Both indole derivatives form 1 : 1 complexes with γ CD, the binding constants of the complexes being different more than twice. Simulation of EPR spectra made it possible to determine the indole ring orientation relative to the plane of the host molecule (at angles in the range 30-60°) and the rotational diffusion coefficients of the complexes, which corresponded to the hydro dynamic volume of one γ CD molecule. In contrast to the complexes with γ CD the rotational correlation times, τ, of the complexes with β CD correspond to a hydrodynamic volume which much exceeds the volume of a single β CD molecule. The complexes with β CD are also characterized by more hydrophobic environment for guest molecules and absence of spin exchange with Ni 2+ ions in the aqueous solution. There results are consistent with a dimeric structure of β CD in the complex and with the orientation of the long axis of the guest molecule along the dimer axis. The energies and geometric parameters were calculated for all complexes by the PM3 method with a conventional set of parameters. The optimized energeti cally stable structures of the 1 : 1 complexes with γ CD and of the 1 : 2 complexes with β CD are consistent with experimental data.
Purpose: to evaluate the clinical efficacy of subthreshold laser treatment of focal DME in continuous and micro-pulse modes using a navigation system based on OCT-A data.Methods. 33 patients (33 eyes) with focal DME were under observation before and after laser treatment. All patients underwent high-resolution OCT-A using the HD Angio Retina 6×6 mm protocol on RTVue-100 XR Avanti (Optovue, USA) and SOLIX (Optovue, USA) devices. The height of edema in the fovea averaged 304.5 ± 5.4 um in the group, in the area of edema outside the fovea — 336.5 ± 7.5 um. The initial best corrected visual acuity (BCVA) was 0.75 ± 0.06. The central retinal sensitivity (CRS) before treatment was 24.10 ± 0.48 dB. The treatment was carried out on the NAVILAS 577S navigation laser system (“OD-OS”, Germany). The results were evaluated within 1, 3 and 6 months after treatment.Results. 1 month after laser treatment, the average CRT in the group was: in the fovea — 294.00 ± 5.38 um, outside the fovea — 318.50 ± 6.44 um. The BCVA increased on average to 0.80 ± 0.06; the CRS to 24.65 ± 0.47 dB. After 3 months, a further decrease in the height of edema was observed (CRT in the fovea was 252.50 ± 2.19 microns, outside the fovea 280.50 ± 3.75 microns). The BCVA increased on average in the group to 0.85 ± 0.06, CRS — 25.50 ± 0.30 dB. After 6 months, the average CRT in the group was: in the fovea — 246.50 ± 1.81 microns, outside the fovea — 273.50 ± 4.56 microns. A stable increase in the BCVA indicators was achieved — 0.85 ± 0.06 and CRS — 26.65 ± 0.16 dB. At the time of 1.3 and 6 months after treatment, an increase in vascular density was observed in the deep vascular complex and the superficial vascular plexus.Conclusion. The preliminary results obtained by us indicate the effectiveness of navigation sub-threshold laser treatment of DME in the micro-pulse and continuous modes, based on targeted topographicallyoriented laser exposure according to OCT angiography.
Background:Obesity is an important health problem, as its prevalence has reached an epidemic level and continues to increase steadily resulting in higher risk of cardiovascular diseases and metabolic disorders. Currently, new methods and criteria are being developed to assess fat and muscle mass, as well as criteria for diagnosing obesity and sarcopenia.Aim:To assess the quantitative composition of muscle and adipose tissue in type 2 diabetes mellitus patients based on the dual-energy X-ray absorptiometry for the diagnosis of obesity and sarcopenia.Materials and methods: We examined 42 type 2 diabetic in-patients admitted to the Department of Therapeutic Endocrinology. Dual-energy X-ray absorptiometry was performed in all patients with subsequent assessment of the composition of muscle and fat tissue.Results:If assessed by the body mass index, all patients had an excess body weight: median, 32.25 [29.75; 35.70]; in men, 31.3 [28.19; 34.63], in women, 32.29 [30.26; 36.54]. 26.2% of the patients (11/42) were overweight, but not obese. Female patients had more severe obesity than male (in total, 33.3% (10/30) of women had 2ndand 3rd degree of obesity, while men 16.7% (2/12)). The assessment by the fat mass index (FMI) showed that 2.4% (1/42) of the patients were normal-weight. Median FMI was 11.91 [10.40; 13.78] (in men, 8.86 [7.46; 12.1], in women, 12.35 [11.55; 15.47]). Overweight was found in 52.4% (22/42) of the patients; in total, 2ndand 3rddegree of obesity was observed in 25% (3/12) of the men and only in 6.6% (2/30) of the women. Median Appendicular Lean Mass Index (ALMI) in the total group was 7.99 [7.32; 9.05], being expectedly higher than in women: 9.19 [8.42; 9.45] and 7.58 [7.24; 8.49], respectively. Median T-score ALMI was 2.32 [1.73; 3.08], Z-score ALMI 2.15 [1.47; 3.54]. In general, there was a decrease in the appendicular muscle mass with age. There was an inverse correlation between the age and T-score ALMI (r = -0.319, р = 0.020), as well as between the age and Z-score ALMI (r = -0.634, p = 0.000). According to the results of T-score ALMI and Z-score ALMI, there were no patients with sarcopenia. However, the calculation of the T- and Z-criteria, corrected for fat mass, has led to a significant decrease of the medians of these parameters and allowed to identify a group of patients meeting the criteria of sarcopenia (97.6%, 41/42).Conclusion:Based on ALMI, T-ALMI, and Z-ALMI, there were no patients with sarcopenia. After these criteria were corrected for fat mass, the number of such patients increased to 97.6% (41/42) and 85.7% (36/42), respectively. The potential use of the adjusted T-ALMI (FMI) and Z-ALMI (FMI) as criteria for sarcopenia and muscle mass reduction compared to the age-related normal values, as well as the classification of obesity by FMI should be studied in large epidemiological studies in different populations.
