Subjective Communication Difficulties in Very Mild Aphasia

Abstract: Purpose In comparison to the large literature on moderate to severe aphasia, very little is known about aphasia of the mildest severity and whether intervention may be warranted. The purpose of this pilot study was to characterize challenges experienced by people who have recovered to the point that they score normally on aphasia batteries yet report they continue to have aphasia. Method Five people with aphasia who scored above the Weste… Show more

“…However, some authors have subsumed it under anomic aphasia [18][19][20], indirectly acknowledging that language abilities in people with latent aphasia are somewhat mildly impaired. Furthermore, recent evidence suggests that despite the within-normal-limits performance in the aphasia test, communication difficulties are reported by such individuals [21]. Similar to other researchers [16], in this study, we define latent aphasia as a diagnostic entity above the cut-off point of the West-ern Aphasia Battery [7].…”

“…Other synonymous diagnostic labels include subliminal aphasia (Boller, 1968), people who have become nonaphasic (Neto & Santos, 2012), not aphasic by WAB [Western Aphasia Battery] (Dalton & Richardson, 2015;Fromm et al, 2017), and very mild aphasia (Cavanaugh & Haley, 2020). The term latent aphasia was coined by Pichot (1955) in the context of people affected by atherosclerosis, although other researchers described similar conditions earlier according to Boller and Vignolo (1968).…”

“…In addition to the careful use of the suggested cutoff criterion scores, clinicians are encouraged to supplement their test interpretation with qualitative observations of their patients' responses. Persons with aphasia exhibit unique behaviours that are not typically observed in neurotypical bilinguals and could help inform differentiating between mild cases of aphasia and low proficiency bilingual behaviours (Armstrong et al 2013;Cavanaugh & Haley, 2020). These include linguistic limitations such as frequent rephrasing, slow rate of speech, filled pauses, prolonged time response, word-finding strategies and paraphasia in order to reliably identify individuals with mild aphasia.…”
