Subconjunctival silicone oil drainage through the Molteno implant

Abstract: To describe silicone oil drainage from the vitreous cavity to the subconjunctival space through a Molteno implant. A 52-year-old aphakic man with a Molteno implant inserted after lensectomy, vitrectomy and intravitreal silicone oil injection visited our hospital. The Molteno tube was located in the anterior chamber with two silicone oil drops on it. Corneal dellen, induced by marked bleb-like elevation of the conjunctiva was noted during follow-up and excision biopsy was attempted. The conjunctival elevation c… Show more

“…4 Glaucoma drainage implants offer an alternative surgical option but oil migration can occur through the tube into the subconjunctival space inciting an inflammatory reaction. 15,16 Cyclodiode treatment can provide good IOP control as reported by Bloom et al 17 and Han et al 18 with success rates of 66 and 82% respectively. Our data indicate a long-term success rate of 74% at 1 year and In the present study, 17 patients (45%) had silicone oil in situ at the time of first cyclodiode treatment, 15 (39.5%) patients had oil in situ at 1 year and 13 (34%) patients at the final follow-up visit.…”

“…Nguyen et al reported 60% success rate with Molteno implant and failure in 1 eye [106]. Inferior tube placement is preferred to prevent silicone oil blocking the tube [107]. …”

“…There are several reports of intraocular silicone oil migration into the subconjunctival space [1, 2, 3, 4, 5], the orbit [5, 6, 7] and even the central nervous system [8, 9, 10, 11]. However, to our knowledge, there are only two reports, describing three cases in total, with intraocular silicone oil migration into the upper eyelid causing ptosis as a postoperative complication [12, 13].…”
