Migration of Intraocular Silicone Oil from the Vitreous Cavity into the Upper Eyelid Causing Ptosis

Abstract: Introduction: To report a case in which intraocular silicone oil migrated into the upper eyelid and caused ptosis. Methods: A 65-year-old woman presented with proliferative vitreoretinopathy in the right eye. Vitrectomies, injection of silicone oil and encircling were performed. Two months after the last operation, swelling of her right eyelid occurred. Result: Magnetic resonance imaging revealed moisture in the palpebral fat tissue. We incised the bulbar conjunctiva and confirmed silicone oil leakage from the… Show more

“…Within the English ophthalmic literature, we found 7 other cases of silicone oil migration to the eyelid from intraocular silicone oil use. 6,[8][9][10][11][12] Our patient showed more extensive disease than the others published, which was consistent with the diagnosis of diffuse siliconoma. We also are the first to illustrate the associated pathological intracytoplasmic, clear vacuoles engulfed by macrophages using electron microscopy.…”

“… 2 Additionally, several prior reports have detailed the possibility of subconjunctival migration of silicone oil, presumably through sclerotomy wounds, leading to eyelid swelling, ptosis, and silicone granuloma. 2 , 3 , 4 , 5 , 6 In one case, subarachnoid migration of silicone through an optic pit was observed. 7 Intracranial migration of silicone without optic pits has also been reported in the literature.…”

“…Intraocular silicone oil is associated with a number of complications such as glaucoma, cataract, subretinal migration of silicone oil, and proliferation of epiretinal and subretinal fibrosis, keratopathy, and in rare situations the migration of silicone oil into the orbit. Migration of silicone oil into the orbit and upper eyelid was also reported after retina surgery following pars plana vitrectomy due to proliferative diabetic retinopathy [ 6 ], endogenous endophthalmitis [ 13 ] and penetrating eye trauma [ 14 ].…”

“…These materials are inert and nontoxic. There are several reports on the migration of intraocular silicone oil into the subconjunctival space [ 1 , 2 ], orbit [ 3 , 4 ], upper eyelid [ 5 , 6 , 7 ] and central nervous system [ 8 ].…”
