Subarachnoid Hemorrhage: Beyond Aneurysms

Abstract: For the purposes of this review, we classify SAH into three main patterns, defined by the distribution of blood on unenhanced CT: diffuse, perimesencephalic, and convexal. The epicenter of the hemorrhage further refines the differential diagnosis and guides subsequent imaging. Additionally, we review multiple clinical conditions that can simulate the appearance of SAH on CT or MRI, an imaging artifact known as pseudo-SAH.

“…When there is clinical suspicion, the imaging method of choice ( • " Table 3) is CCT with CTA [15,16]. In non-enhanced CCT, the subarachnoid space appears hyper-dense in cases of SAH, whereas aneurysmal SAH is typically localized in the basal and ambient cisterns ( • " Fig.…”

“…Other causes of SAH should not be missed when the presence of an aneurysm has been ruled out, like cerebral vein thrombosis, amyloid angiopathy, or rupture of an intracranial vascular malformation [37]. It is noteworthy that in case of brain arteriovenous malformation (bAVM) rupture, the SAH is often associated with an intraparenchymal hemorrhage [38].…”

“…The impairment of cerebral autoregulation and decreased blood flow results in delayed ischemic injury. Angiographic vasospasm is identified in 70% of the patients while 40% have clinical vasospasm, and delayed ischemic injury is seen in 30% of the patients (21,48).…”
