Subantimicrobial Dose Doxycycline Enhances the Efficacy of Scaling and Root Planing in Chronic Periodontitis: A Multicenter Trial

Abstract: Adjunctive subantimicrobial dose doxycycline enhances scaling and root planing. It results in statistically significant attachment gains and probing depth reductions over and above those achieved by scaling and root planing with placebo.

“…Currently, however, the only FDA-approved MMP inhibitor is the tetracycline analogue doxycycline (Periostat Õ ) for the treatment of periodontal disease. [289][290][291] The major obstacle hindering the development of MMP inhibitors for clinical use in patients is our lack of knowledge and understanding of the complex MMP biology and the role MMPs play in CNS disorders such as cerebral aneurysm, epilepsy, AD, and PD. Nonetheless, there is a wealth of preclinical data that support MMP inhibition as a treatment strategy in MS, stroke, and brain cancer.…”

“…Additionally, SDD inhibits the activation of MMPs precursors [10]. SDD has high affinity to bone and soft tissues, thus, its accumulation in the periodontium would further enhance its curative effect [15]. Chio et al [8] reported similar results after administration of SDD that was associated with significant decrease in MMP-8 level in their patients' GCF.…”

“…SDD has been used as an adjunct to standardscaling and root planning (SRP) for the management of periodontal diseases for different treatment periods. Some studies declared significant improvements over those achieved by standard scaling alone [14,15]. Moreover, the long-term use of SDD has been shown to be safe and not associated with the development of antibiotic resistance in the periodontal microbial flora [16].…”

“…[15][16][17] Therefore, the aim of this review was to compare the effects of adjunctive use of NSAIDs with nonsurgical periodontal treatment (NSPT) on periodontal disease based on the available literature. Following focused question was addressed: "In patients with periodontal disease, is there an additional benefit of the oral administration of NSAIDs adjunctive with NSPT compared to control in terms of the clinical attachment level (CAL), probing depth (PD), and adverse outcomes in randomized controlled trials?…”
