Study of visceral hypersensitivity in irritable bowel syndrome

Abstract: As altered rectal perception is present in almost all patients with IBS, it might be a reliable biological characteristic of the disease. Alterations in the MC and SP of the intestinal mucosa may be important factors in visceral hypersensitivity.

“…Indeed, the cellularity of the lamina propria,2 34 35 the number of MCs in the colon2 8 9 or the terminal ileum6 7 36 and the numbers of IELs2 have been found to be significantly increased in IBS patients compared with healthy subjects. Interestingly, in several of these studies, these changes were more marked in the ileum or right colon, whereas the rectum often tended to be normal 6 9 34.…”

“…Among these, mast cells (MCs) are important candidates in IBS since they release several potent inflammatory and immune-modulating mediators that can alter nerve4 and muscle function 5. Quantitative assessments have identified infiltrates of MCs in the ileum6 7 and colon2 8 9 of IBS patients. It has also been shown that MC infiltration and release of mediators in the proximity of mucosal innervation may contribute to abdominal pain perception in IBS patients 8.…”

“…The manifestations of a decreased pain threshold and increased sensitivity of the rectum in IBS patients have been confirmed by large amounts of clinical and experimental data. However, the [18] . IBS patients show a significantly reduced tolerance of balloon dilatation in the colon and rectum, displaying increased pain intensity [19] .…”

“…Under electron microscope, there are many higher density granulars in the plasma. Active MMC is the key cell of gastrointestinal disfunction and abnormal sensitivity [12][13][14][15], producing large portions of chemical cytokine, participating in connections with nerve and immunosystems and in signal conduction within the brain-gut axis, and having an important role in intestinal hypersensitivity and dynamic disorder [16][17][18][19]. It could lead to functional abnormal movement-secretion in the intestine, and improve visceral hypersensitivity of pain.…”
