BACKGROUND: Obesity is a universal health issue of the present time. Nearly 2 billion people were estimated to be either overweight or obese in 2020, with nearly 3.4 million deaths worldwide. Proteinuria is now widely known to be a significant predictor of renal pathologies including end-stage renal disease. This study aimed to assess the relationship between the presence of microalbuminuria (MA) in obese individuals. MATERIALS AND METHODS: This cross-sectional study was conducted among patients attending the outpatient department of Jawaharlal Nehru Medical College and Acharya Vinoba Bhave Rural Hospital, Sawangi (Meghe), Wardha. From the subjects meeting the study criteria, selected 150 individuals with BMI ≥ 25 that formed the obese group. Obese individuals were further subdivided as metabolic healthy obese (MHO) and metabolic abnormal obese (MAO) based on metablic syndrome criteria. From the non-obese patients (BMI≤25), one age and gender matched control was selected for each obese subject. All subjects were tested for MA by dipstick method. Data was analyzed using SPSS and Chi-square test was performed to test for statistical significance. RESULTS: The study reflected the association of MA in the groups studied. The metabolic abnormal obese group was noted as having the highest percentage of positive cases (53.7%) of MA, followed by the MHO group (31.3%). A significant association of prevalence of MA was seen in MHO and MAO obese individuals ( P < 0.001). MA was present in the urine samples of 26 (31.3%) obese subjects in the MHO group, 36 (53.7%) in the MAO group, and 8 (5.3%) in the control population. CONCLUSION: Both MHO and MAO subgroups of obese individuals showed higher proportion of MA indicating adverse renal function. Therefore, primary prophylactic measures such as health education and lifestyle modification should be promoted for the obese to reduce their body weight and thereby possibly reduce the risk of future obesity-related renal complications.

Objectives: To estimate the obesity prevalence in a representative sample of community-dwelling older adults in Tabriz, Iran. Design: Cross-sectional study. Setting(s): Tabriz, the capital of East Azerbaijan Province, Iran. Participants: A representative sample of 1071 (514 males and 557 females) community-dwelling older adults aged≥60 years were selected using the probability proportional to size (PPS) sampling method. Then, anthropometric measures were conducted on 1041 subjects (506 males and 535 females). Outcome Measures: Anthropometric measures including weight, height, body mass index (BMI), hip circumference (HC), waist circumference (WC), and waist-to-hip ratio (WHpR) were conducted and used for the evaluation of overweight and obesity. Obesity was determined according to traditional BMI classifications and population-appropriate WC criterion cut-offs for the estimation of central adiposity. Results: The prevalence of overweight and obesity was 37.4%, 95% CI: 34.5 to 40.4 and 34.3%, 95% CI: 31.4 to 37.3, respectively. Obesity was more prevalent in females (46.6%, 95% CI: 36.9 to 57.2) than in males (21.2, 95% CI: 13.5 to 30.3), but the overweight prevalence was greater in males. Moreover, mean BMI was lower in males than in females (26.9±4.2 kg/ m2 vs. 29.9±6.0 kg/m2 ; P<0.001) and significantly decreased with increasing age. Similarly, mean WC was lower in males than in females (99.7±13.6 cm vs. 102.3±14.3 cm; P=0.002) and decreased significantly with age. Conclusions: The high prevalence of obesity in older people highlights the necessity of designing effective healthy lifestyle interventions and national policies to focus on dietary modification and lifestyle changes and promote physical activity to reduce obesity in aged people, particularly in older Iranian women.

Background : Hypertension of long duration especially if not under control leads to damage of retina, kidney, heart and brain. Endothelial dysfunction is considered to be a predominant pathological finding in patients with hypertension. Microalbuminuria , a pathological marker of endothelial dysfunction is frequently elevated in patients with established essential hypertension and is a predictor of a higher risk for cardiovascular disease and renal dysfunction. The presence of microalbuminuria has been shown to correlate with the other cardiovascular risk factors commonly seen in hypertensive patients. This fact indicates that the detection of an increased urinary albumin excretion could be an index of an increased global cardiovascular risk in a given patient. The aim of this study is to evaluate urinary microalbumin in essential hypertensives with and without end organ damage. Methodology: A total of 255 subjects between the age group of 20-55 years of age, from both sexes who were attending the Hypertensive clinic and Master health check up programme in Sri Ramachandra Medical College were enrolled for the study .They were grouped as three-Group I being controls, Group II hypertensives without complications and Group III hpertensives with end organ damage. After overnight fasting , blood and spot urine samples were collected. All the biochemical parameters were estimated by standard methodologies. Urinary microalbumin was determined by immuno turbidimetric method and creatinine by alkaline picrate method. Albumin Creatinine Ratio (ACR) was also calculated. Results : Among the hypertensives with end organ damage 71.42 % had microalbuminuria , 23.21 % , Left Ventricular Hypertrophy (LVH) and 4.44 % had Retinopathy. A strong correlation was observed between Systolic blood pressure (SBP) and microalbumin levels in hypertensives with end organ damage. Highly significant correlation was observed between urinary microalbumin level and creatinine in all the groups. Statistical analysis was done using SPSS Software 19.0. Discussion: 71.42% of the hypertensives with end organ damage had microalbuminuria. A highly significant difference was observed in Urinary creatinine and Albumin Creatinine Ratio (ACR). It is evidenced that renal damage is an early sign of target organ damage in essential hypertensives. Conclusion: A significant increase in microalbumin levels in essential hypertensives suggests that monitoring the microalbumin levels would be of a great value in assessing the onset of end organ damage.
