Study of Platelet Diameter Distribution in Patients with Valvular Heart Disease and with Prosthetic Heart Valve Replacement

Abstract: We have studied the platelet count and the megathrombocyte index (MI), or percentage of platelets that have a diameter greater than 2.9 μm, in 100 patients with valvular heart disease. The mean platelet count is significantly lower than in controls (p < 0.0005) and the MI is significantly increased (p < 0.01). No difference was observed in these two parameters according to the variety of the valvular heart disease. 45 of the same patients were studied after prosthetic heart valve replacement. Our results show … Show more

“…These results suggest that dogs with cardiac murmurs have altered platelet function. This is in contrast to some initial reports in the human medical literature in which patients with valvular cardiac disease had platelets in an activated state as defined by increased platelet diameter 20 and increased numbers of spread‐type platelets 21 . However, studies to date in dogs have supported the finding of decreased platelet function in cardiac disease.…”

“…Cardiac disease has the potential to alter platelet function in humans and dogs, but there is conflicting information as to whether hypofunction or hyperfunction is present. In earlier studies in humans with cardiac disease, platelets were noted to be in an activated state 20,21 . The few studies reported in dogs have instead supported platelet hypofunction in cardiac disease 1–5 .…”

“…4 Platelet dysfunction with MR has been reported. 5,6 In a morphology study of valvular heart disease, platelets became larger than 2.9 m in diameter 7 and spread-type platelets (hyperactive platelets) were increased. 8 Platelet life span in humans with mitral valve disease has been measured by radioisotope methods.…”
