Stimulation of the Basal Nucleus of Meynert in Senile Dementia of Alzheimer’s Type

Abstract: The basal nuclei of Meynert are the principal sources of cholinerigic innervation of the cerebral cortex. It has been hypothesized that the depressed cortical glucose metabolic activity in senile dementia of Alzheimer’s type (SDAT) may result primarily from diminished activity and loss of these cells. The present study was designed to test the hypothesis that electrical stimulation of the basal nuclei would bring about clinical improvement and increase cortical glucose metabolic activity in SDAT. An electrode … Show more

“…These trials can be divided into the following 5 categories of surgical approaches: cerebrospinal fluid (CSF) shunting [4,5,6,7], intraventricular infusions [8,9,10,11,12,13,14,15,16], tissue grafting [17,18,19], gene therapy [20,21], and electrical neural stimulation [22,23,24,25,26] (table 1). The specific treatment strategies within each category are described in detail below (table 2).…”

“…In 1984 Turnbull unilaterally implanted a flexible electrode into the left NBM via a frontal approach in a 74-year old patient with clinically moderate AD [22]. Based on animal studies, activation of NBM can enhance the behavioral state and thereby boost a range of mental faculties including memory, attention and perception.…”

“…In a case report published in 1985-prior to the current modern era of DBS-Turnbull et al [19] were the first to target this region, implanting a stimulating electrode in the left NBM in a single patient with AD. Stimulation was applied in a cyclic fashion for 9 months.…”
