Steroid use in idiopathic sudden sensorineural hearing loss

Abstract: Few studies have investigated the effect of steroids on idiopathic sudden hearing loss (ISHL). Recently, Wilson, et al., reported a double blind study demonstrating a statistically significant improvement of hearing using steroids in ISHL. This report, fashioned after the Wilson study, confirms his findings and analyzes the relationship of several factors to the probability of hearing recovery.

“…The concept that autoimmunity might damage the labyrinth resulting in bilateral, progressive sensorineural hearing loss (SNHL) was introduced in McCabe (1979). Autoimmunity has since been proposed as an etiology for Meniere's disease (Hughes et al, 1988), sudden SNHL (Moskowitz et al, 1984), and acute vertigo (Hughes et al, 1985). These and other data provide evidence that immune mechanisms and their associated inflammatory response can cause damage to sensitive auditory and/or vestibulary cells.…”

“…Drugs are commonly delivered systemically, with the hope that they will find their way to their intended inner ear targets in the form and concentration desired, and without serious side effects. Systemic corticosteroids, for example, are used in the otologic management of idiopathic sudden and immune-mediated sensorineural hearing losses (e.g., [51][52][53]). Their clinical usefulness, however, is limited by undesirable side effects arising from the high systemic doses required to achieve sufficient cochlear fluid levels of drug to produce the intended inner ear effects [54].…”

“…A participação de auto-imunidade em doenças do ouvido interno como surdez neurossensorial progressiva (35) , vertigem aguda (36) e doença de Menière (37) foi previamente descrita. Em particular, reatividade contra Hsp de 70 kDa foi reportada em pacientes com surdez neurossensorial progressiva (38,39) .…”
