Standards for Collection, Preservation, and Transportation of Fecal Samples in TCM Clinical Trials

Abstract: BackgroundUnlike chemical drugs with a single or a few kinds of active compounds, traditional Chinese medicines (TCMs)uses herbal formulas composed of numerous kinds of chemical constituents. Therefore, TCM clinical trials require unique and stricter standards for collecting, preserving, and transporting fecal samples than those used for chemical drugs. Unfortunately, there are no special standards for processing fecal samples in TCM clinical trials.MethodsWe invited interdisciplinary experts within TCM clinic… Show more

“…Regrettably, diarrhea stool collection or handling process-as central to etiologic-pathogen isolation, are often not exhaustively discussed. Delays in reporting laboratory results can have a significant impact on patient diagnosis and treatment decisions (15) (16) as factors related to microbe stability, such as time and temperature, are major contributors to false results (17) especially for fastidious organisms such as Shigella . For example, Shigella-positive stool samples stored at room temperature for more than 45 minutes, may be culture negative unless stored at 4 degrees or less (18).…”

“…Second, the control group should be set reasonably and strictly matched with the group of interest TCM syndrome to eliminate confounding factors. Third, to ensure data quality, strict standard operating procedures (SOP) should be set for sample collection, storage, processing and acquisition, such as SOP for the collection, storage, and transportation of fecal samples that conform to the unique features of TCM for fecal samples ( Su et al, 2022 ). Fourth, external validation of biomarkers for potential TCM syndromes should be performed before incorporating them into clinical practice.…”

“…Fecal samples were collected prior to the treatments, at week 1, and at week 4, using the MGIEasy kit (MGI Tech Co., Ltd. China), and following the standard operating procedures. 30 , 31 The analysis of gut microbiota and functional predictions was conducted through DNA extraction, polymerase chain reaction (PCR) amplification, and Illumina MiSeq sequencing, employing 16S Ribosomal RNA (16S rRNA) gene sequencing. The Miseq libraries, known as Illumina sequencing libraries, were created by following the established Illumina library preparation protocol using the NEXTFLEX Rapid DNA-Seq Kit (Bioo Scientific, the United States).…”
