Staging of the Axilla in Breast Cancer and the Evolving Role of Axillary Ultrasound

Abstract: Axillary lymph nodes have long been recognized as a route for breast cancer to spread systemically. As a result, staging of the axilla has always played a central role in the treatment of breast cancer. Anatomic staging was believed to be important for two reasons: 1) it predicts prognosis and guides medical therapy, and 2) it is a potential therapy for removal of disease in the axilla. This paradigm has now been called into question. Prognostic information is driven increasingly by tumor biology, and trials s… Show more

“…Evaluating the risk of upstaging of ADH/DCIS is a popular research topic of great clinical importance. Currently, patients with invasive diseases require axillary nodal staging, 29 and preoperative assessment of upstaging risk could allow patient counselling regarding axillary staging at the time of definitive surgery. In the future, patients with low upstaging risk and low histological grade may even opt for active surveillance without the need for wide excision 12 .…”

“…Before deciding about scalonating treatment (local or systemic) because of a suspicious lymph node image, it is essential to confirm the finding with FNAC or core needle biopsy (CNB) [ 29 ]. The PPV of the US alone is not high enough to directly proceed to ALND without tissue sampling, ranging from 56 to 90% [ 29 , 30 ]. Our data are in accordance; we found a PPV of US of 71%.…”

“…For traditional radical mastectomy, as the standard treatment, axillary lymph node dissection (ALND) is an indispensable part of the operation in patients with axillary negative or positive (1)(2)(3)(4)(5). The impact of ALND is to assist clinicians in planning their treatment and to provide more information on prognosis.…”

“…Nowadays, as the first station lymphatic node receiving lymphatic drainage from primary breast tumors, the sentinel lymph node biopsy (SLNB) has become a standard tool for evaluating axillary lymph node status in early breast cancer. Breast cancer patients who have negative sentinel lymph node (SLN) could avoid ALND (2)(3)(4)(5)(11)(12)(13)(14)(15)(16)(17). However, when limited positive SLN were observed, the best treatment for the armpit depends.…”
