Staging of pelvic endometriosis based on MRI findings versus laparoscopic classification according to the American Fertility Society

Abstract: Although MRI has limitations such as suboptimal depiction of small implants and adhesions, this technique is very useful to guide laparoscopy. Moreover, the optimal concordance (95%) between our proposed MRI staging and the AFS laparoscopic classification demonstrated a new advantage of MRI in preoperative staging of endometriosis.

“…However, laparoscopy does not permit an adequate evaluation of subperitoneal extension in all cases. For vaginal endometriosis, clinical examination is superior to both ultrasound and laparoscopy [9,15,26,27]. We were able to recruit 31 patients with deep pelvic endometriosis giving us an average population size.…”

“…MRI is able to obtain a much larger field of view when compared with US, and it able to detect extrapelvic implants assisting in preoperative staging. 60 MRI's role in the diagnosis and management of endometriosis and the detection of deep endometriosis is superior and results in higher accuracy than CT or US.…”
