Indeterminate adnexal masses at ultrasound: effect of MRI imaging findings on diagnostic thinking and therapeutic decisions

Abstract: Inclusion of MRI in the diagnostic algorithm of the indeterminate adnexal mass allows better differentiation of ovarian lesions resulting in a change of therapeutic decision-making with net cost savings.

“…25 In a recently published large systematic quantitative review of the use of the MRI in the discrimination of invasive and borderline tumors from benign ovarian lesions, a sensitivity of 0.92 and a specificity of 0.85 have been reported for this imaging technique. 26 Owing to its remarkably high reported sensitivity, it seems possible that MRI in combination with expert sonography and the RMI could potentially further ameliorate the diagnostic accuracy in the assessment of an adnexal mass.…”

“…map images] may also be obtained for improved detection and characterization of pelvic pathology (2). Qualitative assessment of signal intensities on diffusion-weighted images has shown significant differences in benign and malignant pathology, with high signal intensity was observed more frequently in malignant lesions (5 …”

“…MRI offers excellent tissue characterization due to high soft tissue contrast and can aid in the diagnosis of such pathologies when incompletely evaluated with pelvic ultrasonography (2). MRI also offers high spatial resolution and multiplanar imaging capability, such that differentiation between nonovarian and ovarian lesions is feasible.…”
