Staging of Early Colorectal Cancers: Magnifying Colonoscopy versus Endoscopic Ultrasonography for Estimation of Depth of Invasion

Abstract: Although, there is a substantial difference in the overall diagnostic accuracies, it is not statistically significant. Therefore, we conclude that magnifying colonoscopy is at least as accurate as EUS for preoperative staging of early CRCs.

“…EUS has been considered useful for tumor staging of colorectal cancers; however, its real clinical impact for deciding on the indication for surgical operation is still not clear [13][14][15]. In the present EUS study, when the change for the third layer (submucosal layer) was 1 millimeter or more the histological SM invasion depth was 4.3 mm.…”

“…Matsumoto et al also demonstrated the diagnostic superiority of EUS (probe-EUS) in their study (negative predictive value for deep invasion of 91% vs. 54%, respectively) [19]. In contrast, Fu et al reported that there was no significant difference between magnifying chromoendoscopy and EUS for the preoperative staging of early colorectal cancer [20].…”

“…A previous study showed that endoscopic ultrasonography had higher diagnostic accuracy than pit patterns based on magnifying endoscopy in T staging [18], whereas another study found that the diagnostic accuracy of those methods was similar [19]. Recently, Mukae et al [20] found that the sensitivity and specificity of endoscopic ultrasonography in estimating the deep invasion depth of early CRC was 90 and 87%, respectively.…”
