Purpose Long-gap esophageal atresia is defined by the impossibility to perform a primary anastomosis after birth. We propose a progressive traction surgery by thoracoscopy performed at intervals of 5-7 days. Methods We describe the traction technique of the esophageal pouches through the thoracoscopic approach and without performing a gastrostomy. We use sliding sutures and clips to prevent esophageal tearing and preservation of the azygos vein. Results In a series of three patients, we have being able to reconstruct the esophagus in a period of 1-3 weeks after birth without complications. Conclusions Repetitive thoracoscopy at shorts intervals in the neonatal period allows a tension-free anastomosis of the esophagus in long-gap type esophageal atresia.

The thoracoscopic repair of esophageal atresia with tracheoesophageal fistula is a complex neonatal minimally invasive procedure. The thoracoscopic approach is now nearing its third decade of experience and but is overall still not widely utilized, only in skilled centers and by experienced surgeons. This article will summarize the recent advancements in technique and knowledge in the thoracoscopic approach to this challenging neonatal congenital abnormality.

Резюме Введение. Первичная реконструкция атрезии пищевода с большим диастазом продолжает представлять собой серьезную хирургическую проблему. Значительное количество вариантов лечения, в том числе с сохранением нативного пищевода или его замены на другие сегменты пищеварительного тракта, демонстрируют сложность лечения этого состояния и отсутствие единого подхода. До недавнего времени все дети с атрезией пищевода, сопровождающейся большим диастазом, требовали временной гастростомии, предназначенной для осуществления питания на тот период времени, когда собственный пищевод подвергался вытяжению. В настоящем исследовании представлен новый лечебный подход, состоящий в том, что торакоскопическая тракционная элонгация пищевода возможна без гастростомии. Материалы и методы. В исследовании представлены двое новорожденных с атрезией пищевода и большим диастазом, которые получали лечение в Центре хирургии новорожденных Ивано-Матренинской детской клинической больницы г. Иркутска на протяжении 2017-2018 гг. Операция наложения внутренних швов выполнялась торакоскопическим способом с последующим отсроченным пищеводным анастомозом, выполненным также с применением торакоскопии. Оценкой этого лечения служили возможность наложения анастомоза, осложнения анастомоза и сроки перехода пациента на вскармливание через рот. Результаты. Длительность тракции пищевода составила 5 и 6 дней соответственно. Сохранение нативного пищевода без использования гастростомии было достигнуто у обоих пациентов. Ни у одного из пациентов не наблюдалось утечки анастомоза. Питание через назогастральный зонд начиналось на 6 и 7 день после операции. Кормление через рот полным объемом становилось возможным на 30 и 35 день после конструирования анастомоза. Стеноз анастомоза развился у обоих больных и потребовал 3 и 4 сеанса баллонной дилатации. Последующее наблюдение на протяжении 12 месяцев продемонстрировало, что пищевой режим пациентов не отличался от сверстников. Выводы. Представленный метод удлинения пищевода без использования гастростомии должен рассматриваться как альтернативная стратегия лечения атрезии пищевода с большим диастазом.
