Squamous Cell Carcinoma of the Buccal Mucosa

Abstract: The purpose of this study was to establish treatment criteria for patients with early-stage squamous cell carcinoma of the buccal mucosa. Thirty-one patients were analyzed in a retrospective fashion. Distribution of patients according to tumor stage was relatively even. Within 5 years recurrent disease developed in nearly 80% of evaluable patients. There was a 100% overall incidence of local disease recurrence for patients with stage I and II tumors treated with wide local excision alone and followed up for mo… Show more

“…Studies from North America and Europe have shown a consistently poor prognosis. Strome et al 21 have reported tumor recurrence in nearly 80% of 31 patients within 5 years. Pop et al 22 showed a local recurrence rate of 45% for 40 patients with buccal cancer and 70% for 10 patients with stage IV disease.…”

“…This falls within the 29% to 100% incidence reported in the literature. 15,21,23 A previous report from Taiwan on 57 patients concluded that postoperative radiotherapy was effective in decreasing locoregional failure in patients with close surgical margins, tumor thickness greater than 10 mm, high-grade tumors, and positive node and bone invasion. 13 Of the patients in this study, the S group and S + RT group had similar treatment outcomes in both early and advanced stages despite the fact that 20 of the 58 patients who had S + RT had positive or close surgical margins, whereas no patient in the S group had positive margins.…”

“…6,14,19,20 Strome et al 6 reported nearly 80 per cent of patients having recurred by five years with most (72 per cent) recurrences in the buccal mucosa. In this study most patients underwent Sx alone (61 per cent), with only 29 per cent receiving Sx and adjuvant RT.…”

“…As a result there is a high propensity for lymphatic spread through the rich lymphatic drainage to the jugulodigastric, submental and submandibular lymph nodes. 6 The subsequent development of nodal metastases is a powerful predicator of poor outcome.…”

“…Buccal SCCA was prevalent in Southeast Asia and India because of tobacco use and the chewing of betel nuts. As buccal carcinoma frequently demonstrated aggressive behaviour, and the buccal mucosa had few anatomical barriers, buccal SCCA could easily extend to neighbouring intraoral subsites and adjacent structures, [2][3][4] which affected its T-staging, surgical resectability and radiation planning.…”
