Objectives: Traube's space, used to detect spleen enlargement, is at the precordial area on the anterior wall of the chest. In this respect, correct assessment of this area during physical examination is very important. The aim of this study was to evaluate the area of Traube's space by percussion that is known to be sensitive and specific over 70% and assess effects of gender and defecation habit on the space area.Methods: Thirty-four males and 32 females participated in the study, examined by the same physician. Traube's space was determined on the chest wall by percussion. Images of Traube's space were drawn on a transparent paper using certain reference points for each participant. All images were scanned and measurements were done by AutoCAD 2004 software. Weight and height of the individuals were also measured and their defecation information for the morning they participated in the study was also inquired. Results:In the males, weight, height, body mass index (BMI) and Traube's space values were significantly higher than those of females (p<0.05). There was a positive, strong and significant correlation between Traube's space and weight and height for the whole study group (p<005). There were no statistically significant differences between these two groups when morning defecation was considered (p>0.05). Conclusion:The results of this study may be useful for the evaluation of Traube's space and spleen enlargement during physical examination.

Traube's space is located in the left half of the front chest wall and is limited by the sixth rib, left anterior axillary line, and left costal arch. [1] Within this space lies the fundus of the stomach. [2] During physical examination by percussion, a tympanic sound is produced due to the air-filled structure (Traube open). [3,4] However, in some pathological conditions, the space between the fundus of the stomach and the diaphragm shrinks and the sound heard changes to a dull tone (Traube closed). [3,5] Traube (1818-1876), who performed studies in this field, pointed out that the tympanic sound of this space may not be produced in various diseases. [1,6] In the following years, various related physical examination methods were developed, such as Nixon's method and Castell's method. Nixon studied this space by making patients lie on their right side, whereas Castell studied this space by making the patients lie on their back (supine position). [7,8] The aim of this study was to evaluate the relationship between Traube's space as evaluated by Castell's method and the body mass index (BMI). Materials and MethodsThis cross-sectional study was carried out with the participation of 35 individuals (21 males, 14 females) aged 22 to 30 years. The Ethics Committee of Bülent Ecevit University, Zonguldak, Turkey also approved the study (2008/08 -05.06.2008). Individuals taking part in the study were first given written and verbal information regarding the study. Arq Bras Endocrinol Metab vol 45 nº 6 Dezembro 2001 584 RESUMO RESUMOObesidade é uma doença complexa, de múltiplas etiologias e suficientemente comum para constituir um problema de saúde pública, assim como um dilema clínico importante. Especialistas da área clínica e cirúrgica são freqüentemente desafiados pelas alterações fisiopatológicas associadas com a obesidade. Essas alterações comprometem virtualmente todos os sistemas do organismo, podendo apresentar-se como barreiras no diagnósti-co e na terapêutica. Há evidências marcantes de que obesidade acarreta risco excessivo para a saúde; de fato, a mortalidade aumenta de forma aguda quando o índice de massa corporal ultrapassa 30 kg/m 2 , principalmente quando há distribuição central de tecido adiposo concomitante. O autor discute os efeitos da obesidade nos sistemas respitatório, cardiovascular, digestório e geniturinário. Os obstáculos enfrentados em procedimentos diagnósticos ordinários, prescrição de medicamentos, traumas, assim como descrições patológicas raras de lipomas, são apresentados. ABSTRACT ABSTRACTObesity is a complex and multifactorial disease, common enough to constitute a serious medical and public health problem. Medical and surgical specialists are often challenged by pathophysiological changes associated with obesity. These changes, that compromise virtually all organ systems, may constitute barriers to the progress in diagnosis and therapeutics. There is overwhelming evidence that obesity carries excess risk; in fact, mortality rises sharply when body mass index surpasses 30 kg/m 2 , particularly with concomitant central distribution of adipose tissue. The author discusses the effect of obesity on respiratory, cardiovascular, digestive and genitourinary systems. The obstacles faced by physicians in common diagnostic procedures, drug prescription, traumatic injuries, as well as rare pathologic reports of lipomas, are presented.
