Study design: Case report. Objective: To report a rare lesion of intramedullary cervical tuberculoma. Setting: Department of Neurosurgery, Southwest Hospital, Third Military Medical University, Chongqing, The People's Republic of China. Case report: A 15-year-old boy presented with pain in the neck and shoulder. Examination showed mild left hemiplegia and pinprink hyperalgesia on the left side up to C3 level. Chest X-ray and CT scan showed patch and dot lesions indicating old tuberculous lesions and MRI showed a ring-enhancing intramedullary lesion at C3/4 level. Tuberculoma resection and antituberculous therapy was given. There was satisfactory recovery clinically. Conclusion: Intramedullary cervical tuberculoma is a rare, coexisting systemic tuberculosis and MRI scan helps in obtaining a correct diagnosis; combining surgery with anti-tuberculosis therapy is the treatment of choice.

Nous rapportons un cas de TIM, chez un patient de 48 ans, sans antécédents particuliers, qui a consulté pour un déficit moteur des 2 membres inférieurs d'installation progressive. L'examen clinque a permis d'objectiver un syndrome de compression médullaire thoracique. La découverte d'une masse intra médullaire, après les explorations neuroradiologiques (myéloscanner et IRM), nous a fait poser l'indication d'une exérèse micro-chirurgicale. Le diagnostique de TIM a été affirmé par l'examen anatomo-pathologique de la pièce opératoire. L'association d'une chimiothérapie anti-tuberculeuse au delà de 6 mois après l'exérèse chirurgicale, à permis une guérison complète après un recul de 18 mois. SUMMARYThe intra-medullary tuberculoma (IMT) is a rare tuberculosis located in the central nervous system. We report a case of IMT in a 48-year-old man with a progressive paresis of the lower limbs. The patient had previously been healthy. The neuroradiologic investigations showed an intra-medullary mass. A total exeresis was performed via a micro-surgical approach. The IMT diagnosis was confirmed by the anatomopathological examination.The association of an anti-tuberculosis chemotherapy for 6 months after the surgery led to a complete healing after an 18 months lapse.

J Korean Neurosurg Soc 44 : [36][37][38][39] 2008 Although tuberculosis of the central nervous system is well known, the incidence of intramedullary tuberculomas is low and a combination of intramedullary with intracranial tuberculomas is extremely rare. We report a case of disseminated tuberculoma involving brain and spine with miliary pulmonary tuberculosis in a 66-year-old woman initially presenting with fever, general weakness, back pain and motor weakness of both lower extremities. Despite medical therapy, she developed progressive motor weakness of both lower extremities with muscle strength 1/5 in both lower extremities. Urgent surgical intervention was followed and her muscle power and motor functions were improved gradually. The antituberculous drugs were continued and the follow-up magnetic resonance imaging (MRI) of brain and spine showed that the lesions had become smaller or disappeared.
