Spectrum of MRI Appearance of Mayer-Rokitansky-Kuster-Hauser (MRKH) Syndrome in Primary Amenorrhea Patients

Abstract: The rudimentary uterine buds associated with MRKH syndrome always maintained caudal relationship with ovary. Ovaries or rudimentary uterine buds can be ectopic and should be recognized with MRI before undergoing fertility treatment. Following MRI diagnosis, surgery allows patients to have sexual function with possible attainment of reproduction after assisted reproduction technique or surrogacy.

“…5 In the past decade, a number of studies have studied rudimentary Mullerian remnants in MRKH patients. [5][6][7][8][9][10][11] Presence of rudimentary uterine buds was documented by both the readers with good interobserver agreement. Uterine buds were mostly bilateral.…”

“…18,19 Reporting this fact remains important, to prevent torsion, incarceration and ischaemia of these ectopic inguinal ovaries. 10 Excessive mobility of ovaries and weak suspensory ligaments have been hypothesized as possible causes for inguinal location of ovaries. [19][20][21] Renal anomalies are known to be one of the commonly associated non-gynaecologic anomalies in Type II MRKH syndrome.…”

“…However, due to technical difficulties, the results can sometimes be inconclusive [6]. MRI is the imaging modality of choice for the confirmation of diagnosis and is also useful in the identification of any associated malformations [7,8]. It is worth mentioning that Preibsch et al have shown an excellent correlation between MRI and laparoscopy findings in patients with MRKH syndrome [9].…”

“…MRI can depict the distance of an obstructed vagina from the perineum and the thickness of a vaginal septum or atretic segment. MRI examination can accurately measure the gap between the neck and rectum of the urethra and provide references for the selection of surgical methods [ 21 ]. However, MRI could confirm the pelvic mass derived from ovaries but could not distinguish between polycystic ovary and multilocular ovarian serous cystadenoma.…”
