PurposeTo develop a new visual field simulation model that can recreate real-world longitudinal results at a point-wise level from a clinical glaucoma cohort.MethodsA cohort of 367 glaucoma eyes from 265 participants seen over 10.1 ± 2.5 years were included to obtain estimates of “true” longitudinal visual field point-wise sensitivity and estimates of measurement variability. These two components were then combined to reconstruct visual field results in a manner that accounted for correlated measurement error. To determine how accurately the simulated results reflected the clinical cohort, longitudinal variability estimates of mean deviation (MD) were determined by calculating the SD of the residuals from linear regression models fitted to the MD values over time for each eye in the simulated and clinical cohorts. The new model was compared to a previous model that does not account for spatially correlated errors.ResultsThe SD of all the residuals for the clinical and simulated cohorts was 1.1 dB (95% confidence interval [CI]: 1.1–1.2 dB) and 1.1 dB (95% CI: 1.1–1.1 dB), respectively, whereas it was 0.4 dB (95% CI: 0.4–0.4 dB) using the previous simulation model that did not account for correlated errors.ConclusionsA new simulation model accounting for correlated measurement errors between visual field locations performed better than a previous model in estimating visual field variability in glaucoma.Translational RelevanceThis model can provide a powerful framework to better understand use of visual field testing in clinical practice and trials and to evaluate new methods for detecting progression.

Perimetry, or visual field test, estimates differential light sensitivity thresholds across many locations in the visual field (e.g., 54 locations in the 24–2 grid). Recent developments have shown that an entire visual field may be relatively accurately reconstructed from measurements of a subset of these locations using a linear regression model. Here, we show that incorporating a dimensionality reduction layer can improve the robustness of this reconstruction. Specifically, we propose to use principal component analysis to transform the training dataset to a lower dimensional representation and then use this representation to reconstruct the visual field. We named our new reconstruction method the transformed-target principal component regression (TTPCR). When trained on a large dataset, our new method yielded results comparable with the original linear regression method, demonstrating that there is no underfitting associated with parameter reduction. However, when trained on a small dataset, our new method used on average 22% fewer trials to reach the same error. Our results suggest that dimensionality reduction techniques can improve the robustness of visual field testing reconstruction algorithms.

Glaucoma is the first major category of irreversible blinding eye illnesses worldwide. Its leading cause is the death of retinal ganglion cells and their axons, which results in the loss of vision. Research indicates that glaucoma affects the optic nerve and the whole visual pathway. It also reveals that degenerative lesions caused by glaucoma can be found outside the visual pathway. Diffusion tensor imaging (DTI) is a magnetic resonance imaging (MRI) technique that can investigate the complete visual system, including alterations in the optic nerve, optic chiasm, optic tract, lateral geniculate nuclear, and optic radiation. In order to provide a more solid foundation for the degenerative characteristics of glaucoma, this paper will discuss the standard diagnostic techniques for glaucoma through a review of the literature, describe the use of DTI technology in glaucoma in humans and animal models, and introduce these techniques. With the advancement of DTI technology and its coupling with artificial intelligence, DTI represents a potential future for MRI technology in glaucoma research.
