Sources of diagnostic discrepancies in fine‐ needle aspiration of the breast

Abstract: Fine-needle aspiration (FNA) is a valuable technique to use in the evaluation of breast lesions; however, inadequate and discrepant diagnoses do occur. To identify the source and nature of inaccuracies related to the method we studied 39 cases in which FNA posed diagnostic problems. These problems could be attributed to sampling errors (71.8%), to the criteria of adequacy we use at our institution (25.6%), and to interpretation (2.6%). The nature of the breast lesion (68%) was the most common cause of inadequa… Show more

“…In Kreuzer's study, fibrosis was more common in false negative cases (72%) and cytologically doubtful cases (65%) than in cytologically malignant cases (35%). 4 We also found, as did others, 4,11,18,21 that cytologic diagnosis of large (≥ 5 cm) and diffuse tumors was difficult and resulted in a higher proportion of unsatisfactory samples. The reasons for a lower concordant rate in large tumors were insufficient samples due to hemorrhagic necrosis, vascularity or fibrosis.…”

“…Aspirates of lipoma are usually categorized as unsatisfactory because of absence of ductal epithelial cells. 21 In our study, one lipoma and one angiolipoma were reported as unsatisfactory. When aspirates in our series were mixed with fluid or blood, we made cell block sections, which provided complementary diagnostic information in some cases.…”

“…2,3 Some require a minimum of six groups of ductal cells in addition to bipolar naked nuclei to consider a breast FNA specimen adequate. 4 Others specify that the epithelial cells be present on more than one slide 5 and be obtained from a minimum of three to four needle passes. 6 These criteria, however, have not been extensively tested and pertain predominantly to palpable lesions.…”

“…Apart from marked desmoplasia, the characteristic infiltrative pattern of LC, which is attributed to a paucity of fibronectin, 17 may contribute to the low cellular yield. Several studies have addressed the issue of how cellular yield is influenced by such factors as the number of needle passes, 10,18 tumor size, 8,10,[18][19][20][21] nature of the lesion, 16,19,22 aspirator number 23,24 and interpreter experience. 10,23,24 In the present study, two needle passes were attempted in most cases.…”
