ObjectiveThis study aimed to analyze the results of hysterosonography performed prior to in vitro fertilization (IVF) and to correlate anomalous findings with hysteroscopy.MethodsFindings from 197 hysterosonograms of patients examined in an assisted reproduction clinic between January 2012 and August 2014 were included. Enrollment criteria: patients in preparation for IVF not recently submitted to uterine examination through hysterosalpingography or hysteroscopy referred to hysterosonography. Uterine cavity evaluation was considered anomalous when one or more of the following were found: polyps, submucous myomas, uterine synechiae, Müllerian duct anomalies. Individuals with cavity abnormalities that might interfere with IVF results were referred to hysteroscopy.ResultsNormal test results were seen in 170/197 of the cases (86.3%). Eighteen of the 197 cases (9.1%) were suspected for polyps, two (1%) for submucous myoma, six (3.5%) for synechiae, and one (0.5%) for Müllerian duct anomalies. Sixteen of the patients diagnosed with abnormalities underwent hysteroscopy to confirm or treat the suspected pathology. In only two cases there was no agreement between tests: one patient suspected for synechiae and another for polyps were not confirmed; another individual suspected for polyps was found to have focal endometrial thickening in hysteroscopy. The positive predictive value (PPV) in our study was 93.7%.ConclusionIn most cases, the diagnoses obtained by hysterosonography showed normal uterine cavities. The most common anomalous findings were polyps, followed by synechiae, submucous myoma, and Müllerian duct anomalies. Hysterosonography is a good option for evaluating the uterus and offers a high positive predictive value, while hysteroscopy stands as the gold standard.

BACKGROUNDFemale infertility is a big social and medical problem; however, its diagnosis and treatment is cumbersome. Uterine, endometrial and ovarian pathologies often remain undiagnosed. Diagnostic Laparoscopy (DL) is fast emerging to fill this gap.The aim of this study is to evaluate the role of DL in evaluation of female infertility. MATERIALS AND METHODSA total of 80 infertile female patients (50 primary and 30 secondary) were enrolled in the study. Demographic and clinical history was taken. All the patients underwent laparoscopy followed by D&C. Data was analysed using Chi-square test. RESULTSMajority (60%) were of secondary infertility cases aged >30 years as compared to only 11 (22%) of primary infertility cases. Clinically 8 (16%) of primary infertility and 46.7% of secondary infertility cases were asymptomatic. Endometrial biopsy was performed. In 68 cases (42 Primary, 24 secondary) revealed abnormal pathologies in only 2 (4.8%) cases with primary infertility. However, diagnostic laparoscopy revealed tubular and ovarian abnormalities in majority of cases. Tubal and periovarian adhesions were seen in 24 (30%) cases, hydrosalpinx (bilateral and unilateral) in 24 (30%), ovarian abnormalities were seen in 60% cases. Tuberculosis, endometriosis and fibromyoma were seen in 20%, 7.5% and 1.25% cases. Diagnostic laparoscopy thus provided useful information about tubal, ovarian and endometrial pathologies which otherwise remained undiagnosed. CONCLUSIONDiagnostic laparoscopy showed a high efficacy in diagnosis of causes of female infertility.

[straipsnis ir santrauka lietuvių kalba; santrauka anglų kalba] Intrauterininės sąaugos – tai jungiamojo audinio formavimasis gimdos ertmėje. Šio proceso metu progresuoja endometriumo funkcijos sutrikimas, kuriam būdingi įvairūs klinikiniai simptomai, tokie kaip menstruacinio ciklo sutrikimai (hipomenorėja, dismenorėja ar amenorėja), pasikartojantys savaiminiai nėštumo nutrūkimai, nevaisingumas ir kt. Klinikinė intrauterininių sąaugų raiška yra vadinama Ašermano sindromu.Šio straipsnio tikslas – pristatyti Ašermano sindromo klinikinį atvejį, apžvelgti naujausius mokslinės literatūros šaltinius ir įvertinti problemos aktualumą. Pateikiamas 30 metų amžiaus pacientės klinikinis atvejis. Pacientė planine tvarka kreipėsi į gydytoją akušerį-ginekologą dėl nevaisingumo, kurio trukmė buvo 2 metai. Kitų nusiskundimų pacientė neišsakė, iš anamnezės buvo žinoma, kad moteris negimdžiusi, prieš dvejus metus pacientei buvo atliktas 8 savaičių trukmės nėštumo nutraukimas. Pirminės apžiūros metu, atlikus transvaginalinę (2D) sonoskopiją, gimdos ir gimdos priedų sonoskopinių pakitimų nenustatyta. Intrauterininės sąaugos pacientei diagnozuotos atlikus hidrosonografiją, atliktas papildomas tyrimas – HyFoSy (histerosalpingosonoskopija, angl.Hysterosalpingo Foam Sonography). Pacientei buvo atlikta operacija – histeroskopija, kurios metu pašalintos intrauterininės sąaugos. Pooperacinis laikotarpis buvo sklandus. Praėjus dviem mėnesiams po operacijos pacientei nustatytas nėštumas (pacientė konsultuota gydytojo akušerio-ginekologo). Nėštumo eiga buvo sklandi, cezario pjūvio operacijos metu gimė gyvas, vyriškos lyties naujagimis, pagal Apgar balų skalę įvertintas 9/10 balų.Pagrindinis etiologinis veiksnys, lemiantis intrauterininių sąaugų formavimąsi, yra tiesioginis endometriumo pažeidimas. Pateikiamos keturios intrauterininių sąaugų klasifikacijos sistemos, pagal kurias galima numatyti gydymo taktiką ir komplikacijų riziką bei įvertinti pacientės vaisingumo prognozę. Įtariant Ašermano sindromą, rekomenduojama atlikti sonoskopiją arba hidrosonografiją, histerosalpingosonoskopiją arba histerosalpingografiją ir histeroskopiją. Pagrindiniai gydymo etapai: diagnostinė histeroskopija, histeroskopinis intrauterininių sąaugų suardymas ir profilaktinės priemonės (balioninio kateterio, intrauterininės spiralės įvedimas, hialurono rūgšties gelio ar SprayGelʼio vartojimas), pooperacinė hormonų terapija ir, esant indikacijų, pagalbinis apvaisinimas. Kamieninių ląstelių transplantacija į gimdos ertmę, tikėtina, ateityje taps alternatyva chirurginiam gydymui. Nėštumą po Ašermano sindromo gydymo būtina vertinti kaip didelės rizikos nėštumą.
