Some methodological and statistical issues in the study of change processes in psychotherapy

Abstract: As the number of psychotherapies with demonstrated efficacy accumulates, an important task is to identify principles and processes of change. This information can guide treatment refinement, integration, and future development. However, the standard randomized control trial (RCT) design can limit the questions that can be asked and the statistical analyses that can be conducted. We discuss the importance of examining the shape of change, in addition to identifying mediators and moderators of change. We suggest… Show more

“…HLM is an excellent tool for analyzing trajectories of change over time in designs that feature repeated outcome measures; it can be used to study group differences in trajectories and to detect possible mediators of intervention effects in these designs. 41 Previous work has shown the usefulness of understanding dynamic trajectories of the caregiver experience after SCI 8 and stroke, 10 and its unique value in studying the effects of PST with caregivers over time. 24 Model estimates were obtained using restricted maximum likelihood estimation implemented with the mixed linear models module of SPSS.…”

“…Whereas this approach provides a global account of the efficacy of the treatment, it neglects important intermediate steps of therapeutic change. For example, Laurenceau, Hayes, and Feldman (2007) state that psychotherapy researchers, in addition to understanding what treatments work, are interested in studying how and why treatments work. To begin to answer these questions, it is vital to understand how and when change occurs during treatment rather than waiting until treatment is over.…”

“…Assessing symptoms over such short intervals is necessary to detect the pattern of change of symptoms over the course of treatment (DeRubeis et al, 1990;Laurenceau et al, 2007). The purpose of this study, therefore, was to examine the pattern of improvement in cognitive versus vegetative symptoms of major depression as a function of the form of treatment in a sample in which symptoms were measured repeatedly and relatively frequently.…”
