Introduction: Identifying patients at high risk of developing kidney disease could lead to early clinical interventions that prevent or slow disease progression. Soluble urokinase plasminogen activator receptor (suPAR) is an inflammatory biomarker thought to be involved in the pathogenesis and development of kidney disease. We aimed to determine whether elevated plasma suPAR measured at hospital admission is associated with incident kidney disease in patients presenting to the emergency department.

Liver cirrhosis is the terminal stage of most chronic liver conditions, with a high risk of mortality. Careful evaluation of the prognosis of cirrhotic patients and providing precise management are crucial to reduce the risk of mortality. Although the liver biopsy and hepatic venous pressure gradient (HVPG) can efficiently evaluate the prognosis of cirrhotic patients, their application is limited due to the invasion procedures. Child-Pugh score and the model for end-stage liver disease (MELD) score had been widely used in the assessment of cirrhotic prognosis, but the defects of subjective variable application in Child-Pugh score and unsuitability to all phases of liver cirrhosis in MELD score limit their prognostic values. In recent years, continuous efforts have been made to investigate the prognostic value of body fluid biomarkers for cirrhotic patients, and promising results have been reported. Since the collection of fluid specimens is easy, noninvasive, and repeatable, fluid biomarkers can be ideal indicators to predict the prognosis of cirrhosis. Here, we reviewed noninvasive fluid biomarkers in different prognostic functions, including the prediction of survival and complication development.

To σηπτικό σύνδρομο προκαλείται όταν εξελικτικά σταθερές δομές των μικροοργανισμών (pathogen-associated molecular patterns, PAMPs) συνδέονται σε διαμεμβρανικούς ή ενδοκυττάριους υποδοχείς των κυττάρων της μη ειδικής ανοσίας και ιδιαίτερα των μονοκυττάρων του αίματος και των ιστικών μακροφάγων. Η σύνδεση απολήγει στην απελευθέρωση φλεγμονωδών και αντιφλεγμονωδών κυτταροκινών οι οποίες επάγουν την κλινική εικόνα της σήψης. Η πιο καλά μελετημένη PAMP είναι η ενδοτοξίνη (LPS) του κυτταρικού τοιχώματος των Gram-αρνητικών βακτηρίων η οποία συνδέεται με τον διαμεμβρανικό υποδοχέα τύπου Toll (TLR-4) των μονοκυττάρων και μακροφάγων. Σήμερα είναι γνωστό ότι σε συνέχεια στην έκθεση στη βακτηριακή LPS τα μονοκύτταρα και μακροφάγα εισέρχονται σε μία κατάσταση ανοχής κατά την οποία αν εκτεθούν εκ νέου σε LPS αδυνατούν να παράγουν ικανή ποσότητα φλεγμονωδών κυτταροκινών. Το πολλαπλό τρύμα αποτελεί ένα από τα συχνότερα αίτια θανάτου ασθενών που νοσηλεύονται σε Μονάδες Εντατικής Θεραπείας (ΜΕΘ). Οι ασθενείς αυτοί εκδηλώνουν συνήθως κατά την αρχική εισαγωγή τους στη ΜΕΘ σύνδρομο συστηματικής φλεγμονώδους αντίδρασης (SIRS) μη βακτηριακής αιτιολογίας. Οι ασθενείς που επιβιώνουν από το μετα-τραυματικό SIRS αναπτύσσουν συχνά σοβαρή σήψη ως συνέπεια βακτηριακής επιλοίμωξης. Η εκδήλωση του αρχικού μετα-τραυματικού SIRS οφείλεται σε σύνδεση ενδοκυτταρίων συστατικών του ανθρωπίνου σώματος που απελευθερώνονται μετά τον τραυματισμό με τους υποδοχείς τύπου Toll. Παραδείγματα τέτοιων συστατικών είναι το ουρικό οξύ, το μιτοχονδιακό DNA και μη ιστονικές πυρηνικές πρωτεϊνες τα οποία είναι γνωστά σαν «αλαρμίνες».Σε αναλογία με το φαινόμενο της ανοχής στη βακτηριακή LPS θα ήταν αναμενόμενο τα κύτταρα της μη ειδικής ανοσίας των πολυτραυματιών να εκδηλώνουν ανοχή στις αλαρμίνες οπότε όταν εκτεθούν εκ νέου σε βακτηριακά PAMPs να μην απελευθερώνουν ικανό φορτίο κυτταροκινών. Αν ισχύει αυτό, θα πρέπει οι πολυτραυματίες όταν εισέρχονται σε σοβαρή σήψη να εμφανίζουν ευνοϊκότερη πρόγνωση συγκριτικά με τους ασθενείς που εισέρχοναι σε σοβαρή σήψη χωρίς να έχουν προηγηθεί πολλαπλοί τρυματισμοί. Η παρούσα μελέτη επιχειρεί να αποδείξει την ισχύ αυτής της υπόθεσης μέσα από μελέτες τόσο σε ζωϊκά πρότυπα όσο και σε πολυτραυματίες ασθενείς.
