Peritonitis remains, despite the diagnostic and therapeutic developments, a serious condition, the main cause of septic shock with lethal potential in abdominal surgery Identifying and characterizing the dynamics of early biomarkers of the inflammatory response in abdominal sepsis is important to better follow-up evolution and postoperative infectious complications. The paper aims to study the dynamics of serum concentration of biomarkers LBP, IL6 and bilirubin in patients with acute peritonitis of appendicular cause, in the pre and postoperative stage and the correlation with the clinical evolution of inflammatory syndrome. A prospective 6-month study was performed, including patients for appendicular peritonitis. LBP, IL6, bilirubin, leukocytes were sampled preoperatively, then postoperatively at 72 h, and repeated at 72 h until values returned to normal. Clinical evolution and leukocytes were correlated with biochemical parameters. The mean preoperative values for the studied biochemical parameters were for Il6 of 14.81 +/- 4,047 pg / mL, for LBP of 33,826 +/- 5.5.02 microgram / mL and for bilirubin of 1.77 +/- 0.55mg / mL. The postoperative values decreased in all cases, but remained above normal limits in cases with septic complications. LBP, Il 6 and bilirubin are biomarkers useful in the pre- and postoperative evaluation of sepsis in appendicular peritonitis, correlating more accurately with the evolution of the inflammatory response than leukocytosis. Bilirubin is useful in monitoring intraperitoneal infection, but is not influenced by the existence of extraperitoneal infection. The dynamics of LBP is the most accurate description of bacterial infectious factor exposure.

Liver cirrhosis is the final stage of chronic liver disease (CLD) and is associated with high morbidity and mortality. Various complications such as portal hypertension, ascites retention, hepatic encephalopathy, and hepatorenal syndrome deeply affect patient outcome. The most common tools to predict the outcome of a CLD patient include the following: assessing severity of portal hypertension; scoring systems such as the model of end‐stage liver disease and Child–Pugh score and blood biomarkers related to complications and/or survival rate. In this article, we summarize recent studies of noninvasive markers for predicting impending complications related to CLD and discuss the clinical value of currently available blood biomarkers based on evidence from the literature. In addition, noninvasive blood biomarker assays for different prognostic functions were validated on 113 liver cirrhosis patients at our institution using Kaplan–Meier curve analysis to confirm that these markers can satisfactorily predict CLD‐related patient death.

Liver cirrhosis (LC) is the 11th leading cause of death globally, and its prevalence has been increasing in recent decades. 1 Because of the high fatality rate associated with LC, there have been many attempts to identify accurate prognostic models that will help make better clinical decisions, but these attempts have not led to satisfactory results. 2,3 Histopathologic findings and hepatic venous pressure gradient may reflect disease severity, but their clinical use is limited due to their invasiveness. 4 Widely used noninvasive parameters include the Child-Pugh score and Model for End-Stage Liver Disease (MELD) score-the MELD system is noted for its use in liver transplant allocation. 5,6 These two models use serum bilirubin as a component of their prognostic scales. Most previous studies have used total bilirubin (TB), rather than direct bilirubin (DB), as an indicator of bilirubin metabolism. This paper presents a study that focuses on DB and evaluates its relevance in predicting poor outcomes.Lee et al. 7 compared the prognostic performance of DB and TB in patients with LC. They retrospectively assessed 983 patients who were admitted to the hospital during the study period; the patients were divided into two groups: a training set and a validation set. Lee et al. evaluated significance using the C-statistics for survival and compared the performance of MELD score and DB-MELD score (MELD score determined using DB instead of TB). The results showed that the prognostic performance of DB was higher than that of TB, and DB-MELD score showed a higher area under the receiver operating characteristic curve than the MELD score.
