Solitary Hemorrhagic Cerebellar Metastasis from Occult Papillary Thyroid Microcarcinoma

Abstract: To our knowledge, this is the first reported case of a solitary CM from an occult PTC, and also the first that developed an acute cerebellar hemorrhage years after cranial surgery, however, exhibiting the longest reported survival. This case highlights the importance of not only an appropriate initial treatment of the CM and primary PTC in these patients, but also a close follow-up, to avoid further complications and improve their prognosis.

“…To date, only thirteen cases of CBM from PTC have been published ( Table 1 ) [ 1 12 ]. In agreement with previously reported cases, CBM cases in our series were seen during the sixth and seventh decades of life with mean age of 67 years (range: 64–72), and mean duration between initial diagnosis and CBM was 49.6 months (range: 37–61) [ 1 , 2 , 4 6 ]. In our series, one patient with aggressive variant of PTC (tall cell) was identified.…”

“…Only sporadic cases of hemorrhagic brain metastasis have been reported in association with papillary thyroid carcinoma. [10][11][12]15) The present case demonstrates that metastatic papillary thyroid carcinoma should be included in the differential diagnosis of solitary hemorrhagic tumor in the cerebellum. Early recognition of intracranial tumor should prompt immediate institution of a treatment plan.…”

“…By contrast, solitary hemorrhagic cerebellar metastasis from papillary thyroid carcinoma is extremely rare. [10][11][12]15) We describe a case of a patient with metastasis of papillary thyroid carcinoma to the cerebellopontine angle associated with intratumoral hemorrhage, which was initially misdiagnosed with simple cerebellar hemorrhage, resulting in delay of treatment.…”

“…Cerebellar metastases are even less common. Only 12 cases of metastatic changes in the cerebrum from DTC were reported in the literature, some of them occurring as solitary distant metastasis [2,3,[13][14][15][16][17]. The metastatic lesion in our patient was not solitary.…”
