Solitary Chest Wall Tuberculosis Mimicking a Malignancy Demonstrated on FDG PET/CT

Abstract: Solitary chest wall tuberculosis is rare. Herein, we reported a case of image findings of chest wall tuberculosis in a 44-year-old woman. On FDG PET/CT, it presented as a solitary soft tissue mass with invasion of adjacent rib, diaphragm, and avid FDG uptake with an SUVmax of 10.19, mimicking a malignancy. The postoperative pathology supported a diagnosis of tuberculosis infection.

“…[ 18 F]FDG PET/CT also allowed us to exclude relapse in cases of postoperative fibrosis in patients with abnormal CT results, and this observation is similar to the one made in previous reports [11,18]. When describing the role of [ 18 F]FDG PET/CT imaging in the diag nosis of patients suspected of relapse it should be mentioned that this whole-body examination enables to detect not only distant metastases but also other synchronous conditions [37][38][39][40][41]. It was confirmed in our study.…”

“…Tuberculosis after CAR T-cell therapy was also reported in a letter to editor in which a 9-year-old girl developed tuberculosis 5 months after CAR-T therapy 10 . It is well-known that tuberculosis infection can lead to false-positive in interpretation of 18 F-FDG PET/CT images 11–15 . Tuberculosis infection usually slowly develops, and thus it is unusual to have symptomatic tuberculosis infection within 1 month of CAR-T therapy in a patient who was not infected before the treatment.…”

“…10 It is well-known that tuberculosis infection can lead to false-positive in interpretation of 18 F-FDG PET/CT images. [11][12][13][14][15] Tuberculosis infection usually slowly develops, and thus it is unusual to have symptomatic tuberculosis infection within 1 month of CAR-T therapy in a patient who was not infected before the treatment. Awareness of such possibility is important in accurately interpreting FDG PET/CT images when it is used to monitor efficacy of CAR-T therapy.…”
