Aims The onset of the COVID-19 pandemic saw the suspension of centre-based cardiac rehabilitation (CBCR) and has underscored the need for home-based cardiac telerehabilitation (HBCTR) as a feasible alternative rehabilitation delivery model. Yet, the effectiveness of HBCTR as an alternative to Phase 2 CBCR is unknown. We aimed to conduct a meta-analysis to quantitatively appraise the effectiveness of HBCTR. Methods and results PubMed, EMBASE, CENTRAL, CINAHL, Scopus, and PsycINFO were searched from inception to January 2021. We included randomized controlled trials (RCTs) comparing HBCTR to Phase 2 CBCR or usual care in patients with coronary heart disease (CHD). Out of 1588 studies, 14 RCTs involving 2869 CHD patients were included in this review. When compared with usual care, participation in HBCTR showed significant improvement in functional capacity {6-min walking test distance [mean difference (MD) 25.58 m, 95% confidence interval (CI) 14.74–36.42]}; daily step count (MD 1.05 K, 95% CI 0.36–1.75) and exercise habits [odds ratio (OR) 2.28, 95% CI 1.30–4.00)]; depression scores (standardized MD −0.16, 95% CI −0.32 to 0.01) and quality of life [Short-Form mental component summary (MD 2.63, 95% CI 0.06–5.20) and physical component summary (MD 1.99, 95% CI 0.83–3.16)]. Effects on medication adherence were synthesized narratively. HBCTR and CBCR were comparably effective. Conclusion In patients with CHD, HBCTR was associated with an increase in functional capacity, physical activity (PA) behaviour, and depression when compared with UC. When HBCTR was compared to CBCR, an equivalent effect on functional capacity, PA behaviour, QoL, medication adherence, smoking behaviour, physiological risk factors, depression, and cardiac-related hospitalization was observed.

The global burden of cardiovascular diseases (CVD) is a worldwide public health problem. In 2019, 18.6 million people died from CVD, representing a 17.1% increase compared to 2010. Also, some individuals who experience a cardiovascular event will require some form of cardiovascular procedure, such as a pacemaker or implantable cardioverter-defibrillator insertion, aneurysm repair, or heart valve replacement. Mobile health (mHealth) is a valuable tool for supporting individuals with CVD in self-management, providing medical recommendations, virtual consultations, reminders, and disease monitoring notifications. The main objective of this research was to enhance postoperative care for cardiac procedures. To achieve this, the research involved the development of a new mHealth application and the subsequent evaluation of its usability. The study constituted technological and usability research by using Design Science Research Methodology (DSRM). The design of the mobile application followed the principles of Persuasive Systems Design (PSD) model, which encompass a clear definition of the main task, user interaction through dialogue, system credibility, and social support, aiming to help change user behavior. The sample was non-probabilistic for convenience, and System Usability Scale (SUS) was applied to physicians and nurses as well as individuals in the information technology field. The sample comprised 18 participants, of whom 55.6% were female. The participants rated the application positively, with a median final SUS score of 95 (IQR 90–97.5). Finally, the mobile application presented high usability and user acceptance.

Telemedicine in Surgical Care in Low‐ and Middle‐Income Countries: A Scoping Review

Background Access to timely and quality surgical care is limited in low- and middle-income countries (LMICs). Telemedicine, defined as the remote provision of health care using information, communication and telecommunication platforms have the potential to address some of the barriers to surgical care. However, synthesis of evidence on telemedicine use in surgical care in LMICs is lacking. Aim To describe the current state of evidence on the use and distribution of telemedicine for surgical care in LMICs. Methods This was a scoping review of published and relevant grey literature on telemedicine use for surgical care in LMICs, following the PRISMA extension for scoping reviews guideline. PubMed-Medline, Web of Science, Scopus and African Journals Online databases were searched using a comprehensive search strategy from 1 January 2010 to 28 February 2021. Results A total of 178 articles from 53 (38.7%) LMICs across 11 surgical specialties were included. The number of published articles increased from 2 in 2010 to 44 in 2020. The highest number of studies was from the World Health Organization Western Pacific region ( n = 73; 41.0%) and of these, most were from China ( n = 69; 94.5%). The most common telemedicine platforms used were telephone call ( n = 71, 39.9%), video chat ( n = 42, 23.6%) and WhatsApp/WeChat ( n = 31, 17.4%). Telemedicine was mostly used for post-operative follow-up ( n = 71, 39.9%), patient education ( n = 32, 18.0%), provider training ( n = 28, 15.7%) and provider-provider consultation ( n = 16, 9.0%). Less than a third ( n = 51, 29.1%) of the studies used a randomised controlled trial design, and only 23 (12.9%) reported effects on clinical outcomes. Conclusion Telemedicine use for surgical care is emerging in LMICs, especially for post-operative visits. Basic platforms such as telephone calls and 2-way texting were successfully used for post-operative follow-up and education. In addition, file sharing and video chatting options were added when a physical assessment was required. Telephone calls and 2-way texting platforms should be leveraged to reduce loss to follow-up of surgical patients in LMICs and their use for pre-operative visits should be further explored. Despite these telemedicine potentials, there remains an uneven adoption across several LMICs. Also, up to two-thirds of the studies were of low-to-moderate quality with only a few focusing on clinical effectiveness.
