Small bowel obstruction and the gastrografin challenge

Abstract: The "gastrografin challenge" has been used for decades in the evaluation of small bowel obstruction (SBO). This type of study involves enteric administration of a water-soluble contrast followed by serial abdominal radiographs. While its diagnostic role is well established, its therapeutic role remains controversial. Following an algorithm for gastrografin challenge cases can help with interpretation. An understanding of the appearance of diluted contrast in the small bowel, the concentrating effect of contras… Show more

“…Gastrografin has an osmolarity of 1900mOsm/L which is six times the osmolarity of the blood, allowing shifting of fluid from the intracellular compartment to the bowel lumen increasing pressure on the obstructive site [13]. It also makes the contents of the bowel dilute and decreases bowel wall edema, favoring the relief of obstruction and increasing intestinal motility [14]. The appearance of contrast medium in the colon within 24 hours of administration signifies partial obstruction and delay in operative treatment can be done.…”

“…Thus, in addition to the possible clinical signs of ischemia, such as fever, leukocytosis, tachycardia, and/or evidence of peritonitis, contrast material administration did not decrease the need for surgery in patients with uncomplicated acute adhesive SBO (61). A randomized trial is needed to determine whether the water-soluble contrast material test has therapeutic effects (62). Recommendations on the contribution of this test to the diagnosis of adhesive SBO are conflicting.…”
