Sleeve Gastrectomy—A “Food Limiting” Operation

Abstract: Our study indicates the constant effect of SG in the acceleration of gastric emptying of solids, which occurs faster, not only in short but also in long-term postoperatively. Such effects on gastric motility, in combination with the reported alterations in gut hormones, may explain how this 'food limiting' operation results in weight loss.

“…Melissas et al 7 showed that solid gastric emptying before and after LSG (with morbidly obese patients acting as their own controls) was accelerated. Braghetto et al 8 confirmed this in their subsequent study, showing acceleration for liquids and solids (78 -38 min).…”

“…Ghrelin is predominantly secreted by the gastric fundus and has various effects on food intake, such as increasing appetite and influencing chewing time, taste preferences, and food perception, as well as increasing gastrointestinal motility and decreasing insulin secretion. In non-obese individuals, ghrelin levels are highest immediately prior to a meal and decrease rapidly upon the initiation of food intake (13). Conversely, obesity is associated with reduced ghrelin levels, and food intake fails to modulate the levels of plasma ghrelin.…”

“…Conversely, obesity is associated with reduced ghrelin levels, and food intake fails to modulate the levels of plasma ghrelin. Dietaryinduced weight loss is linked to increased ghrelin secretion, which has been postulated to contribute to the poor long-term success rates of dietary weight loss programmes by promoting increased food consumption, although this remains unproven (12,13).…”

“…This phenomenon depends on the increased speed of gastric emptying after surgery and results in an increase in GLP1 secretion by L cells. 92,93 Recent studies have shown that vertical gastrectomy as an isolated and definite bariatric surgery is as effective as the Fobi-Capella procedure for the control of comorbidities; in addition, it provides the potential to avoid some of the associated complications. [94][95][96][97] Although the surgical procedure is simpler, research investigating humans who were undergoing vertical gastrectomy alone demonstrated the presence of limitations 98 and complications.…”

“…One can infer that the metabolic benefits obtained by such procedures are almost exclusively due to ileal interposition. Note that the increased speed of intestinal transit caused by vertical gastrectomy 92,93 may contribute to the more rapid arrival of undigested food in the region of ileal segment interposition and thus contribute to the production of GLP-1 by L cells in the ileum. Nevertheless, the isolated ileal interposition has proven to be effective in correcting dysmetabolism in several animal studies.…”
